Auswirkungen eines von Physiotherapeuten gesteuerten individualisierten Hybridmodells der Übungskomponente der Herzrehabilitation auf die Patientenergebnisse: eine prospektive Kohortenstudie

Detaillierte Beschreibung
Zweck: Das Ziel dieser prospektiven Kohortenstudie bestand darin, die Wirksamkeit eines individualisierten, von einem Physiotherapeuten (PT) gesteuerten Hybridmodells der Übungskomponente der Herzrehabilitation (CR) zu bestimmen, das ein neuartiges Intensitäts-Erholungs-Progressionsprotokoll (IRPP) und Herztests verwendet Vorlage (CTT) zur Beurteilung der erreichten Herzfrequenzintensität (AHRI%), der Herzfrequenzerholung (HRR%) und der Intensitätserholung insgesamt (IR%) als Indikatoren zur Steuerung der Behandlung.
Methoden: Auswertung eines subjektiven Fragebogens, Vitalparameter, 6-Minuten-Gehen auf dem Laufband (6MW), 1-Minuten-Aufstehen (1STS), 1-Minuten-Schritt (1ST) und 1-Minuten-Agility-Quadrattests (1AST) wurden an 100 Teilnehmern während des 36 Besuche umfassenden Programms bewertet. Das Hybridmodell wurde von einem Physiotherapeuten geleitet, hatte eine längere Dauer, fügte hochintensives Intervalltraining (HIIT) zu kontinuierlichem Aerobic-Training (CAET) und Krafttraining hinzu und verwendete ein Behandlungsverhältnis von 1:1. Behandlungsintensität und -fortschritt wurden anhand der klinischen Entscheidungsfindung mittels IRPP und CTT gemessen.

Beschreibung
Einschlusskriterien:
- Hatte eine Herzoperation (CABG, PCI mit Stentplatzierung und Klappenersatz)
- Die Operation erfolgte innerhalb der letzten 365 Tage
- Konnte selbstständig in die Einrichtung gehen.
Ausschlusskriterien:
- Implantierbarer Kardioverter-Defibrillator (ICD)
- Herzschrittmacher (PPM)
- Herz Transplantation
- Linksherzunterstützungsgerät (LVAD)
- Alle anderen Herzverfahren, die nicht in den Einschlusskriterien aufgeführt sind
- Diagnose: Vorhofflimmern
- Teilnehmer, die zuvor eine kardiologische Rehabilitation besucht haben
