Helicobacter pylori est la principale cause de gastrite chronique, d'ulcère peptique et de tumeurs gastriques (adénocarcinome et lymphome). La guérison de l'infection à H. pylori prévient la récidive de l'ulcère duodénal et gastrique et améliore la dyspepsie dans une proportion importante de cas, elle est donc rentable.
La thérapie d'éradication a changé au fil du temps. Le schéma habituellement recommandé dans les conférences de consensus est traditionnellement la trithérapie, composée de l'association de 2 antibiotiques (clarithromycine plus amoxicilline ou métronidazole) et d'un inhibiteur de la pompe à protons (IPP) pendant 7 à 14 jours. Des méta-analyses récentes indiquent que le taux mondial actuel d'éradication après une trithérapie standard est inférieur à 80 %. Plusieurs études européennes ont trouvé des taux d'éradication encore plus faibles, avec 35 à 40 % des cas entraînant un échec du traitement. L'échec du traitement entraîne un deuxième traitement et un nouveau test diagnostique pour confirmer l'éradication.

Justification de l'étude :
L'une des dernières innovations thérapeutiques est la thérapie séquentielle, introduite en Italie par Zullo et al. La stratégie d'administration du médicament consiste en une phase d'induction de 5 jours avec une bithérapie (un IPP toutes les 12 heures et amoxicilline 1g toutes les 12 heures), suivie immédiatement d'une trithérapie pendant 5 jours avec un IPP toutes les 12 heures, métronidazole 500 mg toutes les 12 heures et clarithromycine 500 mg toutes les 12 heures. La thérapie séquentielle s'est avérée plus efficace que la trithérapie standard pendant sept et dix jours. Afin d'évaluer l'efficacité de ce nouveau traitement dans notre zone, nous avons mené au cours de l'année écoulée une étude pilote observationnelle multicentrique dans les conditions de pratique clinique courante chez des patients infectés par H. pylori et avec des indications d'éradication. 139 patients de 6 centres différents ont été inclus. Une excellente observance du traitement et des effets secondaires minimes, similaires à ceux décrits avec la trithérapie, ont été démontrés. Les résultats confirment une efficacité similaire à celle obtenue dans les études précédentes et un taux d'éradication nettement supérieur à celui obtenu avec la trithérapie classique dans notre région.
Plusieurs études antérieures ont montré une excellente efficacité de la quadruple thérapie avec un IPP, l'amoxicilline, la clarithromycine et le métronidazole, administrant « en concomitance » les mêmes médicaments que ceux de la thérapie séquentielle, mais seulement pendant 5 à 7 jours. Plusieurs études ont trouvé un taux d'éradication plus élevé de cette thérapie « concomitante » par rapport à la trithérapie classique. Les résultats d'une étude randomisée dans laquelle la thérapie séquentielle et concomitante ont été comparées ont récemment été rapportés au Congrès américain de gastroentérologie. Les taux d'éradication et d'événements indésirables étaient similaires avec les deux traitements. Ces données suggèrent que l'administration "séquentielle" de ces médicaments a probablement compliqué inutilement le traitement.
Par conséquent, il est nécessaire de faire un essai clinique contrôlé pour comparer directement la thérapie « séquentielle » par rapport à la thérapie « concomitante ». Les résultats de cette étude concluront quel devrait être le traitement de première ligne pour l'éradication de H. pylori.

Critère d'intégration:
- Inclusion des patients atteints de dyspepsie ou d'ulcère gastro-duodénal pour lesquels un traitement d'éradication est indiqué.
- Exigence de confirmation du diagnostic d'infection à H. pylori par au moins un test positif parmi les suivants : test respiratoire, histologie, test rapide à l'uréase ou culture.
Critère d'exclusion:
- Âge inférieur à 18 ans.
- Maladie chronique avancée ou toute autre pathologie qui empêche d'assister aux contrôles et au suivi.
- Allergie à l'un des antibiotiques du traitement.
- Chirurgie gastrique antérieure
- Grossesse et allaitement.
- Antécédents d'abus d'alcool ou de drogues.
- Traitement d'éradication antérieur.
- Consommation d'antibiotiques ou de sels de bismuth au cours des 4 dernières semaines
