Questo studio confronterà un inalatore per l'asma che utilizza un nuovo propellente alternativo rispettoso del clima con un inalatore per l'asma con un propellente esistente. Vogliamo assicurarci che entrambe le versioni dell'inalatore funzionino allo stesso modo per le persone con asma da lieve a moderata.

Questo studio globale mira a confrontare se l'inalatore sperimentale con il nuovo propellente HFA-152a è altrettanto sicuro, efficace e ben tollerato nei pazienti con asma rispetto agli inalatori che utilizzano l'attuale propellente HFA-134a. L'HFA-152a è un gas propellente progettato per avere un potenziale di riscaldamento globale inferiore e per essere più rispettoso del clima. Lo studio sarà condotto in Europa, America Latina, Ucraina, Sudafrica e Regno Unito. È prevista la randomizzazione di 468 pazienti.

Descrizione

Criteri di inclusione:

  • Adulti di sesso maschile e femminile (18 ≤ età ≤ 75 anni) con diagnosi di Asma da almeno 6 mesi prima dello screening e con diagnosi prima dei 50 anni;
  • Non fumatori, ex fumatori;
  • Indice di massa corporea: compreso nell'intervallo da 18,0 a 35,0 kg/m²;

  • Terapia per l'asma stabile: un trattamento di mantenimento stabile per almeno 4 settimane prima dello screening con:

    1. basse o medie dosi di ICS (corticosteroidi inalatori) da soli; oppure
    2. basse o medie dosi di ICS + LABA (agonisti β2 a lunga durata d'azione) (combinazione fissa o libera).
  • Asma controllato o parzialmente controllato in base a un punteggio del questionario di controllo dell'asma - 7 voci (ACQ-7) <1,5 allo screening e alla randomizzazione.
  • FEV1 pre-BD (broncodilatatore) >40% e <90% del valore normale previsto, dopo un appropriato washout dai BD, alla visita di screening (V1).
  • Un aumento dimostrato del FEV1 o della capacità vitale forzata di >12% e >200 mL rispetto al basale entro 30 minuti (min) dopo l'inalazione di 400 µg di salbutamolo (cioè albuterolo) pMDI alla visita di screening (V1).

Criteri di esclusione:

  • Storia di asma quasi fatale o ospedalizzazione per asma in unità di terapia intensiva, ricovero in regime di degenza o accesso al pronto soccorso per asma nei 6 mesi precedenti lo screening, che a giudizio dello Sperimentatore potrebbe esporre i soggetti a un rischio indebito;
  • Esacerbazione recente dell'asma che richiede corticosteroidi sistemici (SCSs), o ammissione al pronto soccorso o ospedalizzazione entro 3 mesi prima dello screening e/o durante il periodo di run-in;
  • Asma non persistente: asma da sforzo, asma stagionale (come unica diagnosi correlata all'asma) che non richiede farmaci di controllo giornaliero per l'asma;

  • Soggetti con asma attualmente trattati con uno dei seguenti:

    1. Alte dosi di ICS;
    2. Antagonista muscarinico a lunga durata d'azione (LAMA);
    3. Corticosteroidi sistemici, depot o a lento rilascio entro 12 settimane prima dello screening;
    4. Qualsiasi altro trattamento per l'asma (ad es. sodio cromoglicato, sodio nedocromile, modificatori dei leucotrieni) entro 4 settimane prima dello screening;
    5. Qualsiasi terapia biologica (ad es. omalizumab, mepolizumab, reslizumab, benralizumab, dupilumab, tezepelumab) entro 6 mesi prima dello screening;
  • Disturbi respiratori diversi dall'asma
  • Resezione polmonare;
  • Infezione del tratto respiratorio inferiore;
  • Carcinoma polmonare e storia di carcinoma polmonare;
  • Soggetti con cancro attivo o una storia di cancro (diverso dai polmoni);
  • Pazienti con una condizione cardiovascolare clinicamente significativa;
  • Conformità al run-in: completamento del e-Diario <75% e conformità al trattamento del run-in <75% alla randomizzazione;
