心臓病を持つ子供のケアは進歩しているにもかかわらず、心疾患を持つ子供たちは退院後も複雑なケアを必要とすることがよくあります。 退院時に親が直面するケアのニーズには、複数の薬、経管栄養、医療機器などがあります。 当然のことながら、多くの親が病院から在宅へのケアの移行に問題があると報告しています。 臨床医療従事者は、退院前後の家族教育やコミュニケーションにおける課題に関連して不満を表明しています。 退院後の子供の世話に対する親/介護者の自信を高め、臨床症状の悪化、計画外の30日間の再入院、救急外来(ED)受診、外来予約の不順守など、退院後の意図しないリソースの利用を減らすための介入戦略を特定するための研究が必要である。
研究の目的 1 では、研究者らは、心臓病を患うハイリスク児の退院後のケアニーズを満たすために親や介護者を支援するための、修正可能な障壁、認知されたニーズ、介入の機会を、半構造化された定性面接を実施することによって特定した。親/介護者、医療関係者。 そして、参加者の回答に基づいて、研究者らは、ハイリスクの子供の世話における親/介護者の快適さを改善し、退院後の転帰を改善するためのサポートツールを開発しました。 この研究の目的は、ヌムール心臓センターの 5 つの高リスク家族でこのサポート ツールを試験的に導入し、その経験に基づいて受け入れ可能性と実現可能性を評価することです。

ヌムール心臓センターにケアのために来ている心臓病を患う高リスクの子供の親/介護者には「サポートツール」が提供され、経験を通じてその受け入れと実現可能性を尋ねられます。

退院前介入 -
退院後の介入 (1 ~ 2 日目): • 家族とのビデオ/Facetime 通話で、設備や家庭環境を確認し、薬や粉ミルクに関する質問に答えます。 退院後の介入 (3 ~ 7 日目): • 家族に電話して、最初の再診予約のための交通手段を確保し、薬、酸素、食事についての質問に答えます。 |
