Momenteel wordt cementfixatie van de humeruscomponent aanbevolen, aangezien de zorgstandaard en literatuuronderzoek aangeven dat deze fixatie gepaard gaat met een laag aantal complicaties. Ongecementeerde fixatie is veelbelovend in zijn potentieel om langdurige, stabiele fixatie te bieden, maar vergelijkende resultaten met gecementeerde stengels zijn op dit moment niet beschikbaar.
Ondanks het ontbreken van gegevens om het gebruik ervan te ondersteunen, is er een exponentiële toename in het gebruik van ongecementeerde humeruscomponentfixatie bij schouderartroplastiek. Deze push kan worden ingegeven door de wens om de operatietijd te verkorten in een poging om chirurgische kosten te besparen. Dit doel wordt met name benadrukt door de inspanningen van de overheid en externe instanties om het rendement op beperkte middelen te maximaliseren. Er zijn op dit moment geen sluitende klinische gegevens die suggereren dat ongecementeerde fixatie resultaten oplevert die beter zijn dan gecementeerde fixatie in de schouder.
Deze multicenter, prospectieve, gerandomiseerde, dubbelblinde klinische studie zal gecementeerde versus ongecementeerde/weefselingroeide fixatie van de humeruscomponent bij totale schouderartroplastiek vergelijken. Patiënten worden gerekruteerd uit 10 centra verspreid over Canada door chirurgen met uitgebreide ervaring in totale schouderartroplastiek. Patiënten zullen worden gerandomiseerd om ofwel een gecementeerde of ongecementeerde humerussteel te krijgen. Voor alle andere variabelen wordt gecontroleerd.
De belangrijkste evaluatie van de patiëntuitkomst is de ziektespecifieke kwaliteit van leven. Hiervoor worden gevalideerde tools gebruikt die de schouderfunctie evalueren, zoals vereist door verschillende verenigingen. Deze omvatten de Western Ontario Osteoarthritis of the Shoulder Index (WOOS), de meest responsieve; Constant, de voorkeur van Europese onderzoekers; American Shoulder and Elbow Surgeons (ASES) Gestandaardiseerde schouderbeoordeling en schouderscore-index, bij voorkeur door de ASES. De algehele wereldwijde gezondheidsstatus wordt gemeten met behulp van de SF-36. Tijdens het postoperatieve beloop worden verschillende secundaire uitkomsten gecontroleerd. Dit zijn: schouderfunctie, radiografische evaluatie van componentfixatie, operatietijd en de incidentie van revisiechirurgie en complicaties.

Beschrijving
Inclusiecriteria:
- Primaire artrose van de schouder van graad III of hoger (Kellgren en Lawrence aangepast voor de schouder
- Patiënten bij wie de standaard conservatieve behandeling van hun schouderartrose niet is gelukt
Uitsluitingscriteria:
- Patiënten met secundaire artrose als gevolg van ernstig gewrichtstrauma, infectie, avasculaire necrose, cuff-artropathie, chronische dislocatie, massale scheur in de rotatorcuff, inflammatoire artropathie, arthropathie van Charcot of eerdere schouderoperaties (anders dan artroscopisch debridement)
- Patiënten met preoperatieve CT-scans van de schouder die onvoldoende glenoïdbotvoorraad laten zien waardoor implantatie van een glenoïdprothese niet mogelijk is
- Actieve gewrichts- of systemische infectie
- Aanzienlijke spierverlamming
- Ernstige medische ziekte (levensverwachting minder dan 2 jaar of onaanvaardbaar hoog operatierisico)
- Kan geen Engels/Frans spreken of lezen
- Psychiatrische ziekte die geïnformeerde toestemming verhindert
- Niet bereid om 2 jaar gevolgd te worden
