Lymfodepletie en anti-PD-1-blokkade om terugval te verminderen bij AML-patiënt die niet in aanmerking komt voor transplantatie

Fase II-onderzoek naar lymfodepletie en anti-PD-1-blokkade om terugval te verminderen bij AML-patiënten met een hoog risico die niet in aanmerking komen voor allogene stamceltransplantatie
AML is de meest voorkomende acute leukemie bij volwassenen.
De meeste patiënten kunnen als mogelijke genezing een allogene stamceltransplantatie ondergaan; veel patiënten komen echter niet in aanmerking voor allogene transplantatie vanwege leeftijd, algehele gezondheid, psychosociale factoren en/of gebrek aan beschikbare donoren.
Daarom kunnen deze patiënten niet profiteren van de therapeutische voordelen van het "graft-versus-leukemie"-effect van donorimmuuncellen.
Het doel van deze studie is om hopelijk de immuuntolerantie voor AML-cellen te doorbreken om betere resultaten te bieden bij patiënten met AML met een ongunstig risico.

Gedetailleerde beschrijving
AML-patiënten met een ongunstig risico ondergaan een voorbereidend regime van lymfodepletie van Flu/Mel gevolgd door autologe transplantatie.
Anti-PD-1-therapie van pembrolizumab begint op dag +1 na stamceltransplantatie en wordt elke 3 weken toegediend voor in totaal 8 doses.
Volgens de literatuur wordt het risico op terugval na 2 jaar geschat op 60-80% bij patiënten met een ongunstig risico op AML in CR-1.
Met dit protocol veronderstellen de onderzoekers dat na lymfodepletiechemotherapie en pembrolizumab het terugvalrisico na 2 jaar zal afnemen tot minder dan of gelijk aan 35%.
De eenzijdige Wald-test op significantieniveau van 5% wordt gebruikt om de hypothese te testen.
De grootte van 20 patiënten levert de power van 90,5% op, ervan uitgaande dat de feitelijke 2-jaarsleukemievrije overleving 60% is.

Inclusiecriteria:
- Niet-gunstig risico AML
- In CR-1 of latere CR
- Ten minste één cyclus van consolidatiechemotherapie voltooid
- Verzameling van minimaal 2x106/kg CD34+ cellen
- KPS van 70% of hoger
Uitsluitingscriteria:
- Onderzoeksmiddel ontvangen binnen 4 weken na de eerste dosis
- Voorafgaande chemotherapie, bestralingstherapie binnen 2 weken na de eerste dosis
- Overgevoeligheid voor pembrolizumab of een van de hulpstoffen
- Eerdere therapie gehad met anti-PD-1-, anti-PD-L1- of anti-PD-L2-middelen
