Chloortalidon en bumetanide bij vergevorderde chronische nierziekte: HEBE-CKD-onderzoek
Deze studie heeft tot doel het mogelijke voordeel aan te tonen van een behandeling op basis van dubbel diureticum bij patiënten met chronische nierziekte en een ernstig verminderde glomerulaire filtratiesnelheid. Dit is gebaseerd op eerdere waarnemingen waarbij de onderzoekers ontdekten dat volumeoverbelasting een frequente aandoening is binnen deze populatie en sterk verband houdt met een toename van morbiditeit en mortaliteit. De onderzoekers zijn van mening dat deze therapie gunstig zou kunnen zijn, aangezien de meeste van deze patiënten worden behandeld met lisdiuretica, maar met het verstrijken van de tijd treden adaptieve veranderingen in het distale nefron op die een afname van het behandelingseffect bevorderen. In die zin zouden thiazidediuretica in de juiste dosering de resistentie kunnen 'breken', aangezien hun werkingsmechanisme het resistentiemechanisme tegenwerkt. Helaas is deze behandeling tot op de dag van vandaag niet volledig geëvalueerd. Zeker bij dit soort mensen.
De onderzoekers ontwikkelden een studie die werd voorgesteld als een dubbelblinde, gerandomiseerde klinische studie, waarbij de bevolking in twee groepen zal worden verdeeld. Een groep krijgt de standaardbehandeling op basis van lisdiureticum (bumetanide), terwijl de andere groep de interventie krijgt (bumetanide plus chloortalidon). Na een follow-up periode van 30 dagen worden de resultaten gemeten.
Met betrekking tot de effectiviteit van de behandeling zal de afname van volumeoverbelasting door bio-impedantie worden gemeten. Terwijl het optreden van nadelige effecten tijdens dezelfde monitoringperiode zal worden waargenomen.

Beschrijving
Inclusiecriteria:
- glomerulaire filtratiesnelheid minder dan 30 ml / min / 1,73 m
- Zonder substitutietherapie (dialyse of hemodialyse)
- Volume-overbelasting
- Minstens 100 ml per dag resterende diurese
- Gebruik van een lisdiureticum gedurende minstens een maand
Uitsluitingscriteria:
- Allergieën bekend bij diuretica
- Patiënten met ernstige infecties
- Patiënten met hemodynamische instabiliteit
- Geamputeerden
- Patiënten met cognitieve stoornissen
- Patiënten met acuut nierfalen
- Patiënten met transplantaatverlies

Deze groep krijgt een week lang 3 milligram bumetanide per dag plus placebo (zetmeel) dat de chloortalidondosis van de behandelgroep zal simuleren.
Als de dosis goed wordt verdragen, wordt de dosis bumetanide verhoogd tot 4 milligram per dag.
