Deze studie is een prospectief, multicenter, 1:1 gerandomiseerd klinisch onderzoek om de veiligheid en werkzaamheid van het stentsysteem bij de behandeling van intracraniale aneurysma's te evalueren. In totaal zullen naar verwachting 204 patiënten worden geïncludeerd. Het primaire eindpunt is de incidentie van succesvolle aneurysma-occlusie zes maanden na de procedure. Alle patiënten zullen klinische follow-up krijgen tijdens intraoperatieve periodes, 30 dagen, 6 maanden, 1 jaar.

Inclusiecriteria:

  1. Leeftijd 18 jaar tot 80 jaar.
  2. Wijde hals (wijdhals wordt gedefinieerd als een breedte ≥ 4 mm of een koepel-halsverhouding < 2), sacculaire, intracraniale aneurysma's die voortkomen uit een moedervat met een diameter van ≥ 2,5 mm en ≤ 5 mm.
  3. Gemodificeerde Rankin-schaal (mRS) ≤ 2.
  4. De patiënt begrijpt de aard van de procedure, geeft voorafgaand aan de behandeling vrijwillig schriftelijke geïnformeerde toestemming en is bereid te voldoen aan de gespecificeerde vervolgevaluatie.
  5. De onderzoekers oordeelden dat het gebruik van stent-ondersteunde spiraalembolisatie geschikt was voor de behandeling van intracraniaal aneurysma.

Uitsluitingscriteria:

  1. Doelaneurysma is een aneurysma dat een gefaseerde procedure vereist.
  2. Doelaneurysma dat eerder is behandeld met vasculaire embolisatie of een operatie.
  3. Doelaneurysma's zijn fusiforme aneurysma's, dissectie-aneurysma's, pseudo-aneurysma's, bloedblaasachtige aneurysma's, geïnfecteerde aneurysma's en aneurysma's geassocieerd met arterioveneuze malformaties.
  4. Chirurgische behandeling in het ziekenhuis binnen 30 dagen vóór de operatie, of geplande chirurgische behandeling in het ziekenhuis binnen 6 maanden na de operatie.
  5. Acuut gescheurd aneurysma binnen 14 dagen na inschrijving.
  6. Hunt en Hess (H&H) schaal ≥3.
  7. Proefpersoon met de ziekte van Moyamoya, arterioveneuze malformaties, arterioveneuze fistels, intracraniale tumoren of intracraniale massa die gericht is op aneurysma's.
  8. Ernstige stenose of kronkeligheid van intracraniale slagaders of anatomische afwijkingen waardoor het moeilijk is om de plaats van de laesie met het apparaat te bereiken.
  9. Aantal bloedplaatjes <50*103/mm3 (50*109/L) of een bekende stollingsdeficiëntie, of International normalised ratio (INR)>3,0.
  10. Bekende overgevoeligheid/allergieën of contra-indicatie voor contrastmiddelen, stentcomponenten, bloedplaatjesaggregatieremmers, anticoagulantia.
  11. Levensverwachting < 1 jaar, en niet in staat om de vereiste follow-ups te voltooien;
  12. Vrouwen die zwanger waren of borstvoeding gaven, vrouwen/mannen die van plan waren het komende jaar kinderen te krijgen.
  13. Momenteel ingeschreven voor een ander onderzoeksapparaat of geneesmiddelenstudie.

Experimenteel: SINOMED IAS-stentsysteem
Het SINOMED IAS-stentsysteem is bedoeld voor gebruik met occlusieve apparaten bij de behandeling van intracraniale aneurysma's.
Stent-assisted coiling (SAC) van intracraniale aneurysma's maakte de weg vrij voor endovasculaire coiling van aneurysma's met wijde hals, waardoor de snelheid van aneurysma-vernietiging en recidief werd verbeterd.
Actieve vergelijker: Neuroform Atlas-stentsysteem
Het Neuroform Atlas-stentsysteem is bedoeld voor gebruik met occlusieve hulpmiddelen bij de behandeling van intracraniale aneurysma's.
