Intranasale oxytocine-interventie voor zorgverleners van personen met dementie

Het algemene doel van deze studie is om de werkzaamheid en veiligheid van intranasale oxytocine (OXT) te bepalen om de kwaliteit van leven te verbeteren en chronische stressniveaus bij de zorgverleners van personen met dementie te verminderen. Een gerandomiseerde, dubbelblinde, placebogecontroleerde klinische studie met intranasale oxytocine (12 of 24 IE) zal worden uitgevoerd bij dagelijkse toediening (eenmaal daags) gedurende 21 dagen (3 weken).
Endogeen neuropeptide oxytocine (OXT) is een focus geweest van eerder psychiatrisch onderzoek, gebaseerd op zijn rol in pro-sociaal gedrag en modulatie van de reactie op sociale/emotionele stimuli. Momenteel is er echter geen alomvattend onderzoek naar de rol ervan bij de zorgverleners van chronische medische/psychiatrische ziekten, waaronder dementie, op mogelijke gunstige effecten en op het verminderen van chronische stressniveaus.
Het huidige onderzoek heeft tot doel de werkzaamheid en veiligheid van intranasale OXT voor deze klinische implicaties vast te stellen. We passen ook methoden toe (taakgerelateerde functionele MRI) die een verifieerbaar en meetbaar vermogen hebben aangetoond om het kerndoelgebied (mPFC en amygdala) en de verandering ervan als reactie op OXT vast te leggen.
Dit project zal leiden tot vooruitgang in het begrijpen van de potentiële rol van OXT in de relatie tussen zorgverlener en de persoon met chronische medische/psychiatrische ziekten zoals dementie. Het zal ook de toekomstige richting bepalen voor het klinische gebruik van intranasale OXT voor deze kritische populatie.

Beschrijving
Inclusiecriteria:
- Vrouwen van 50 jaar of ouder
- Momenteel onbetaald mantelzorger voor een oudere volwassene (50 jaar en ouder) met dementie voor minimaal vijf uur per week gedurende minimaal zes maanden.
- Normaal of gecorrigeerd naar normaal zicht en gehoor
- Mobiliteit om naar UNMC te reizen voor onderzoeksprocedures, waaronder beeldvorming van de hersenen; Rechtshandig
- Capaciteit om in het Engels te lezen en te schrijven
Uitsluitingscriteria:
- Ernstige medische ziekte die de toediening van oxytocine (OXT) gecontra-indiceerd heeft (bijv. ernstige leverziekte, convulsies, stofwisselingsstoornissen)
- Voorgeschiedenis van allergische reactie op OXT en zijn neussprayproduct
- Voorgeschiedenis van CZS-ziekte (waaronder voorgeschiedenis van convulsies, epilepsie, CZS-tumor, CZS-bloeding of ernstige CZS-infectie waaronder meningitis of encefalitis)
- Momenteel zwanger of van plan zwanger te worden in de komende weken tijdens het onderzoek
- Metaal in het lichaam (d.w.z. gehoorapparaat, pacemaker, botplaten, beugels, niet-verwijderbare piercings/implantaten, enz.), claustrofobie of enige andere aandoening die MRI-scannen uitsluit.
- Mini-mentale status examenscore van 25 of lager, wat mogelijke cognitieve problemen suggereert
- Geschiedenis van of huidige neurologische ziekte (bijv. beroerte, traumatisch hersenletsel, hersentumor, dementie)
- Geschiedenis van onze huidige ernstige psychiatrische ziekte (bijvoorbeeld schizofrenie, bipolaire stoornis, autisme, ernstige posttraumatische stressstoornis)
- Geschiedenis van of huidig drugs- of alcoholmisbruik
- Momenteel borstvoeding
- Huidige COVID-ziekte
- Linkshandig vanwege structurele verschillen in de hersenen tussen rechts- en linkshandige individuen
- Gebruikt momenteel antiopsychotische medicijnen, SSRI's of corticosteroïdencrèmes/pillen.
