Description détaillée
L'objectif de l'étude est de documenter les résultats de performance clinique et de dispositif du système Evolut R utilisé dans la pratique hospitalière de routine dans une grande cohorte de patients pour le traitement de la sténose symptomatique de la valve aortique native ou d'une défaillance de la valve bioprothétique chirurgicale sténosée, insuffisante ou combinée nécessitant une valve. remplacement.
Étude prospective, à un seul bras, multicentrique, observationnelle, post-commercialisation. En Australie et au Canada, il s'agit d'une étude de pré-commercialisation prospective, à un seul bras et multicentrique.
Jusqu'à 60 centres dans le monde. Les zones géographiques peuvent inclure l'Europe, l'Australie, le Moyen-Orient et l'Afrique, l'Amérique latine et le Canada.
Environ 1 000 sujets implantés ont consenti à un suivi sur trois ans.
Pour chaque sujet, les données seront recueillies en préopératoire, peropératoire, à la sortie de l'hôpital, 30 jours, 1, 2 et 3 ans.

Critère d'intégration:
- Sténose valvulaire aortique native symptomatique ou défaillance valvulaire bioprothétique chirurgicale sténosée, insuffisante ou combinée nécessitant un remplacement valvulaire
- Candidat acceptable pour un traitement électif avec le système Evolut R et en conformité avec le contexte réglementaire et médico-économique local
Âge ≥ 80 ans OU considéré comme présentant un risque élevé ou plus élevé de remplacement chirurgical de la valve aortique (RVA), où le risque élevé est défini comme :
- Risque de mortalité prédit par la Society of Thoracic Surgeons (STS) ≥ 8 % OU
- Accord documenté de l'équipe cardiaque sur le risque de RVA en raison de la fragilité ou de comorbidités.
- Géographiquement stable et disposé à retourner sur le site d'implantation pour toutes les visites de suivi
- Avoir l'âge légal pour fournir un consentement éclairé (consentement éclairé du patient ou formulaire de divulgation des données) dans le pays où ils s'inscrivent à l'essai
- Le patient a été informé de la nature de l'étude, est capable et disposé à donner son consentement sans l'assistance d'un représentant légal et a consenti à participer, et a autorisé la collecte et la divulgation de ses informations médicales en signant un consentement éclairé du patient ou Formulaire de diffusion des données.
Critère d'exclusion:
- Hypersensibilité connue ou contre-indication à l'aspirine, à l'héparine (HIT/HITTS) et à la bivalirudine, à la ticlopidine, au clopidogrel, au nitinol (titane ou au nickel), ou sensibilité aux produits de contraste, qui ne peuvent pas être correctement prémédiquées
- Valve cardiaque mécanique préexistante en position aortique
- Septicémie en cours, y compris endocardite active
- Anatomiquement non adapté au système Evolut R
- Espérance de vie estimée à moins d'un an
- Participer à un autre essai pouvant influencer le résultat de cet essai
- Besoin d'une intervention chirurgicale d'urgence pour quelque raison que ce soit
