Cyclosporine A Plus Traitement à faible teneur en stéroïdes dans la pneumonie COVID-19
Utilité de faibles doses de corticostéroïdes et de cyclosporine associées à l'énoxaparine, chez les patients atteints de pneumonie COVID-19 à l'hôpital régional ISSSTE, Puebla, pendant la période d'urgence due à la pandémie de SRAS-Cov2

Description détaillée
Il s'agit d'une étude pilote réalisée dans un seul centre (hôpital régional ISSSTE, Puebla, Mexique). Des patients consécutifs avec un diagnostic suspecté ou confirmé de COVID-19 ont été assignés, de manière non aveugle et non randomisée, pour recevoir soit des stéroïdes plus CsA (groupe d'intervention), soit des stéroïdes uniquement (traitement standard dans cet hôpital, groupe témoin), selon jugement clinique individuel. Le critère de jugement principal était le nombre de jours d'amélioration clinique jusqu'à la sortie de l'hôpital ou le décès. Le critère de jugement secondaire était l'amélioration des patients, définie par les paramètres suivants : diminution des besoins en oxygène (2 litres par minute ou moins), absence de fièvre pendant trois jours consécutifs, fréquence respiratoire < 22, diminution de 50 % ou plus de la Protéine C réactive à l'admission et durée du séjour à l'hôpital.
Les patients adultes fréquentant l'hôpital entre le 15 avril et le 31 mai 2020, présentant des symptômes compatibles avec la pneumonie COVID-19, selon les directives de l'OMS, ont été invités à participer et tous les patients ont donné leur consentement éclairé écrit.

- Pneumonie COVID 19 confirmée par tomodensitométrie pulmonaire clinique et prélèvements nasopharyngés par RT-PCR.
Critère d'exclusion:
- Infection grippale.

Expérimental: Groupe de contrôle COVID-19 Pneumonie
Patients atteints de pneumonie COVID-19 avec traitement standard : énoxaparine 0,5 mg/kg S.C. une fois, clarithromycine 500 mg deux fois et méthylprednisolone 0,5 mg/kg une fois par voie endoveineuse.

Patients atteints de pneumonie COVID-19 traités par CsA par voie orale à une dose de 1-2 mg/kg/jour divisée en deux doses, pendant 7 jours, depuis leur entrée en hospitalisation.

Amélioration des patients, définie par les paramètres suivants : baisse des besoins en oxygène (2 litres par minute ou moins), absence de fièvre pendant trois jours consécutifs, fréquence respiratoire <22, diminution de 50 % ou plus de la protéine C réactive à l'admission , et la durée du séjour à l'hôpital.
