Cyclosporine A Plus behandeling met lage steroïden bij COVID-19-pneumonie
Nut van lage doses corticosteroïden en ciclosporine gecombineerd met enoxaparine, bij patiënten met COVID-19-longontsteking in het ISSSTE Regional Hospital, Puebla, tijdens de onvoorziene periode als gevolg van de SARS-Cov2-pandemie

Dit is een pilootstudie uitgevoerd in een enkel centrum (ISSSTE Regional Hospital, Puebla, Mexico). Opeenvolgende patiënten met vermoedelijke of bevestigde diagnose van COVID-19 werden, ongeblindeerd en niet-gerandomiseerd, toegewezen aan steroïden plus CsA (interventiegroep) of alleen steroïden (standaardbehandeling in dit ziekenhuis, controlegroep), volgens individueel klinisch oordeel. De primaire uitkomstmaat was het aantal dagen tot klinische verbetering tot ontslag uit het ziekenhuis of overlijden. Het secundaire resultaat was de verbetering van de patiënten, gedefinieerd door de volgende parameters: lagere zuurstofbehoefte (2 liter per minuut of minder), afwezigheid van koorts gedurende drie opeenvolgende dagen, ademhalingsfrequentie <22, een afname van 50% of meer in de C-reactief proteïne bij opname en duur van het ziekenhuisverblijf.
Volwassen patiënten die tussen 15 april en 31 mei 2020 naar het ziekenhuis gingen en volgens de richtlijnen van de WHO symptomen vertoonden die passen bij COVID-19-pneumonie, werden uitgenodigd om deel te nemen en alle patiënten gaven hun schriftelijke geïnformeerde toestemming.

Experimenteel: COVID-19 Longontsteking controlegroep
COVID-19-pneumoniepatiënten met standaardbehandeling: enoxaparine 0,5 mg/kg s.c. eenmaal, claritromycine 500 mg tweemaal en metylprednisolon 0,5 mg/kg eenmaal endoveneus.

Patiënten met COVID-19-longontsteking behandeld met orale CsA in een dosis van 1-2 mg / kg / dag verdeeld over twee doses, gedurende 7 dagen, sinds ze in het ziekenhuis zijn opgenomen.
