この無作為化試験では、入院患者のオピオイド解毒を求めている同意者は、2 つのグループのいずれかに割り当てられます。 最初のグループは、急性禁断症状を緩和するための入院患者の解毒中にブプレノルフィンの漸減スケジュールを受け取り、地域の医療および薬物乱用治療プロバイダー(DETOX)に紹介されます。 2 番目のグループも、禁断症状を緩和するためにブプレノルフィンを投与されますが、退院後 1 週間以内に入院患者の退院とプライマリケア提供者との予約へのリンクを通じてブプレノルフィンの安定した用量を維持します (LINKAGE)。 すべての参加者は、ベースライン、登録後1か月、3か月、および6か月で研究評価を完了します。
マサチューセッツ州フォールリバーに拠点を置くSSTAR入院患者解毒プログラムが、この試験の募集場所として機能します。 SSTAR のプログラムには 38 床の病床があり、医学的および精神医学的に管理された 24 時間体制の治療施設であり、評価と離脱管理を提供しています。 SSTAR ファミリー ヘルスケア センターは、同じくフォールリバーにある 330 の連邦認定施設であり、コミュニティ ヘルス ネットワーク エリア 25 (フォールリバー、サマセット、スウォンジー、およびマサチューセッツ州ウェストポート) の住民にプライマリ ケアと問題行動医療サービスを提供しています。 SSTARの研究ディレクターであるベイリー博士は、解毒中の研究参加者へのブプレノルフィン治療の提供を監督し、ファミリーヘルスケアセンターのプライマリケア提供者への連絡役を務めます。

説明
包含基準:
- 18歳以上;
- オピオイド依存;
- -維持ブプレノルフィン治療の開始に興味があります;
- 尿毒物学でオピオイド陽性。
- SSTARでブプレノルフィンを免除された医師とのプライマリケアを喜んで確立します。
除外基準:
- -急性疾患、認知障害、または精神病のためにインフォームドコンセントを提供できない患者;
- プロトコールへの参加を妨げる可能性のある保護観察/仮釈放要件 (SSTAR 入院患者の 1%);
- 場所を確認するための 2 人の連絡担当者を提供できない。
- 英語で評価を完了することができない。
- 現在、メタドンまたはサブオキソンのメンテナンスプログラムに登録されています。
- 今後 6 か月以内にその地域を離れることを計画している。
- 鎮静/催眠薬、コカイン、またはアルコールに対する物質依存の現在のDSM-IV診断;
- 自殺念慮の修正尺度における現在の自殺傾向;
- 継続的なオピオイドの使用を必要とする慢性的な痛み、または今後6か月以内に予想される主要な痛みを伴うイベント(重大な外科的処置);
- 妊娠;
- ブプレノルフィンまたはナロキソンに対するアレルギー反応の既往。
