現在のデータによると、米国の子供の 54 人に 1 人が自閉症スペクトラム障害 (ASD) と診断されています。 追加のデータは、自閉症と診断された人は、注意欠陥/多動性障害(ADHD)などの併発疾患と診断されるリスクがあることを示しています。 それぞれの症状を管理することは、医療従事者、患者、親にとって困難な場合があります。
ADHD と診断された ASD 患者は、症状を治療するための最適な薬物療法の選択肢を見つけるという課題に直面することがよくあります。 ASD および ADHD の子供は、これらの治療に対して、定型発達 (TD) の子供とは異なる反応を示すことが多いことが知られています。 現在、ASD および ADHD の患者に最適な薬物療法の選択肢を導くために、親や医療提供者が従うべき明確な推奨事項は存在しません。 このような薬の知識の欠如と最適とは言えない投薬管理は、不十分な結果をもたらし、患者とその家族の人生に多大な悪影響を与える可能性があります。
この研究の主な目的は、TD児のADHDに対して最も多く処方されている薬が、自閉症と診断されている患者にも同様に効果があるかどうかを判断することである。 この研究の結果は、ASD患者に対する第一選択の覚せい剤であるメチルフェニデートとアンフェタミン化合物に対する医療従事者の理解を向上させるでしょう。 親は、子供や医師との話し合い、さらには自分自身の観察を通じて薬の有効性を判断し、薬の調整プロセスを推進します。 保護者は 2 週間ごとに臨床全体印象測定を使用して有効性の評価を提供し、異常行動チェックリストとヴァンダービルト保護者評価スケールは毎月完了します。 この研究は、第一選択薬に反応しない患者にとって、第二選択薬の選択肢であるα2作動薬であるグアンファシンとクロニジンについての理解をさらに深めることになるでしょう。 保護者は以前と同じ基準を使用して評価を提供します。 追加の研究措置では、投薬継続を決定する過程で親/介護者の意見を収集し、検討します。 ほとんどの対策は電子ポータルを通じて完了するため、この研究に参加するために家族が病院に来る必要性が減ります。

- 患者および/または法的に権限を与えられた代理人は、この研究への参加についてインフォームドコンセント/同意を与える意思および能力がなければなりません。
- 患者とその介護者は、(治験責任医師の意見では)すべての研究要件に従う意欲と能力がなければなりません。
- 患者は登録時に4歳から17歳まで(両端を含む)でなければなりません。
- 患者は裏付けとなる証拠(例: 参照元医師の報告書、ADOS (自閉症診断観察スケジュール) や CARS (小児自閉症評価尺度) などの医療記録。
- 患者は一貫して薬を服用することができなければなりません(錠剤、液体、または食物/液体と混合して)。
- 患者は、ADHD の確定診断を受けていなければなりません (DSM-5 基準および裏付け証拠に基づく)。
- 患者には、子供と定期的に時間を過ごす一貫した報告者(親など)が必要です。
- 同意前の4週間以上用量が安定しており、研究中に用量を変更する計画がない場合、患者は他の向精神薬(選択的セロトニン再取り込み阻害剤(SSRI)、非定型抗精神病薬、抗けいれん薬)を服用することができます。
- 登録の1年以上前にADHD治療薬の治験を行った患者。以前の治験が失敗した理由についての臨床医の判断に基づく。
- 患者および/または家族/介護者は、この研究に関連するアンケートに回答できるよう、英語またはスペイン語に十分に堪能である必要があります。
除外基準:
- 過去1年以内にADHD治療薬の治験を複数回受けた患者歴。
- 最後に ADHD 治療薬を服用してから 4 週間未満。
患者が治験薬を安全に服用することを妨げる可能性のあるその他の危険因子。
- 参加者の IQ に基づく包含/除外基準はありません。 開業医が IQ 範囲全体にわたる ASD の子供の ADHD を治療するように求められているのと同じように、認知能力の全範囲にわたる個人を対象とします。
