Botmetastasen zijn vaak een van de eerste tekenen van uitgezaaide ziekte bij kankerpatiënten. Skeletcomplicaties als gevolg van gemetastaseerde ziekte omvatten (ernstige) botpijn, verminderde mobiliteit, compressie van het ruggenmerg, pathologische fracturen en hypercalciëmie. Radiotherapie en chirurgie zijn de opties voor de specifieke lokale behandeling van botmetastasen. Chemotherapie, hormoontherapie en bisfosfonaten zijn systemische wapens die worden gebruikt bij de behandeling van botmetastasen met of zonder hypercalciëmie. Kankers met de neiging om uit tezaaien naar botten zoals borst, prostaat, long en myeloom, kunnen het vermogen hebben om te interageren met osteoclasten. Osteoclasten zijn gespecialiseerde botcellen, die gemineraliseerd bot eroderen door zuren en lysosomale enzymen af te scheiden. Bij normale botremodellering is osteoclastische botresorptie gekoppeld aan en in evenwicht met osteoblastische botvorming. De lytische botvernietiging geassocieerd met kwaadaardige botmetastasen ontwikkelt zich doordat tumorcellen oplosbare factoren synthetiseren en vrijgeven die osteoclasten stimuleren om bot te resorberen. De kwaadaardige activatie van osteoclasten resulteert in een verstoring van de normale botremodellering waarbij het evenwicht tussen botresorptie en botvorming verschuift naar verhoogde botresorptie. Deze relatieve toename van osteoclastische botresorptie resulteert in een netto verlies van bot.
Zoledroninezuur (Zometa®, CGP42446) is een lid van een klasse verbindingen die bekend staat als bisfosfonaten. Bisfosfonaten zijn effectieve remmers van osteoclastische botresorptie. Ze hebben therapeutische werkzaamheid bij de behandelingen van hypercalciëmie van maligniteit, lytische botziekte geassocieerd met multipel myeloom, en gemengde lytische en blastische botmetastasen geassocieerd met borstkanker, prostaatkanker en longkanker. In de klinische setting is zoledroninezuur het krachtigste bisfosfonaat.
Conventioneel is uitwendige bestraling (RT) een primaire behandelingsmethode voor de palliatie van botmetastasen. Het doel van RT bij de behandeling van botmetastasen is om kwaadaardige cellen uit te roeien zonder omliggende normale cellen te beschadigen. RT wordt typisch gegeven aan het laesiegebied, om zoveel mogelijk beenmerg te sparen. RT is geïndiceerd bij solitaire, lytische en pijnlijke botlaesies van multipel myeloom, evenals botmetastasen van solide tumoren zoals borst-, prostaat- en longkanker om het risico op fracturen te voorkomen of om de pijn te verlichten.
Het doel van deze studie is het evalueren van de veiligheid en werkzaamheid van gelijktijdige standaard RT en standaard zoledroninezuur op de botmetastasen van borst-, prostaat- of longkankerpatiënten. We kozen voor zoledroninezuur om in deze studie te gebruiken, omdat het het meest effectieve door de FDA goedgekeurde aminobisfosfonaat is.

Inclusiecriteria:
- >18 jaar
- biopsie bewezen borst-, long- en/of prostaatkanker
- botmetastasen met een indicatie voor bestraling, hetzij radiologisch vastgesteld, hetzij biopsie bewezen
- Prestatiestatus Karnofsky >60
- Levensverwachting van minimaal 6 maanden
- Serumcreatininespiegel ≤ 2,0 mg/dL en berekende creatinineklaring van > 60 ml/min
- Aantal leukocyten ≥ 3500/mm3
- Hemoglobine > 11 g/dl
- Bloedplaatjes > 100.000 / mm3
- Totaal bilirubine < 2,5 mg/dl
Uitsluitingscriteria:
- zwangere vrouwen en vrouwen die borstvoeding geven (borst- en longkanker)
- hormoontherapie gestart minder dan 3 maanden voorafgaand aan randomisatie (prostaatkanker)
- voorgeschiedenis van allergische reacties op bisfosfonaten
- gelijktijdig nefrotoxische chemotherapie krijgt
- deelname aan een ander klinisch onderzoek met een geneesmiddel in onderzoek of een geneesmiddelonderzoek in onderzoek binnen de afgelopen 30 dagen voltooid
- leverfunctietesten > 1,5 keer normale waarden
- IV calcitoninetoediening minder dan 30 dagen voorafgaand aan randomisatie
- laboratorium bewijs van nierziekte
- eerdere RT naar het gebied van botmetastasen dat in deze studie zal worden behandeld
- momenteel orale of intraveneuze bisfosfonaattherapie krijgt
- aanwezigheid van ascites
- klinisch significante elektrocardiografische veranderingen
- hypercalciëmie, pathologische fractuur of epidurale compressie van het ruggenmerg
- andere orgaanmetastasen
- bekende overgevoeligheid voor Zometa® (zoledroninezuur) of andere bisfosfonaten
- huidige actieve gebitsproblemen waaronder infectie van de tanden of kaakbot (bovenkaak of onderkaak); tand- of armtrauma, of een huidige of eerdere diagnose van osteonecrose van de kaak (ONJ), blootliggend bot in de mond of langzame genezing na tandheelkundige ingrepen.
