Het voorgestelde onderzoek test een theoretisch en empirisch afgeleide behandelbenadering gericht op het faciliteren van verandering in gezamenlijke aandachtsinteracties tussen verzorgers en hun peuters met autisme. Jonge kinderen met autisme vertonen beperkingen in het aangaan van gezamenlijke aandachtsvaardigheden zoals wijzen en laten zien. Het belang van gezamenlijke aandacht wordt onderstreept door gegevens die aantonen dat deze vaardigheden belangrijk zijn voor de latere taalontwikkeling. Toch zijn deze interactie- en vaardigheidstekorten zelden de focus geweest van systematische interventie-inspanningen, vooral bij zorgverleners. Bovendien zijn de huidige interventies voor jonge kinderen met autisme gedragsgericht, therapeutgestuurd en sluiten ze vaak de laagst functionerende en ontwikkelingsjongste kinderen uit. Het aanpakken van deze tekorten bij jonge kinderen in de ontwikkelingsfase met vertrouwde verzorgers kan dus resulteren in betere taalresultaten voor deze kinderen.
Het overkoepelende doel van het voorgestelde project is het rigoureus testen van een interventieprogramma voor verzorgers en hun peuters met autisme dat op de ontwikkeling is geïnformeerd, kindgericht is en gericht is op gezamenlijke aandachtsinterventie met hun peuters versus moeders die oudervoorlichting krijgen over autisme en de ontwikkeling van het kind.
De hoofddoelen van dit onderzoek zijn als volgt:
- Doel 1: Bepalen of door de zorgverlener gemedieerde interventie op het gebied van gezamenlijke betrokkenheid superieur is aan educatie van de zorgverlener op het gebied van sociale communicatie en taaluitkomsten bij kinderen.
- Doel 2: Bepalen of de ontwikkeling van vaardigheden in de context van interactie met het kind van een verzorger overgaat in interacties met leerkrachten in de klas en leeftijdsgenoten.
- Doel 3: Kenmerken van gezinnen en kinderen onderzoeken die de uitkomst van sociale communicatie het best voorspellen.

Beschrijving
Inclusiecriteria:
- Kind voldoet aan screeningcriteria voor PDD
- Momenteel niet ingeschreven in een reeds bestaand interventieprogramma
- Ouder is beschikbaar voor tussenkomst
- Het kind moet klaar zijn om te beginnen met het Early Childhood Partial Hospitalization Program (ECPHP) aan de UCLA
Uitsluitingscriteria:
- Het kind heeft geen andere sensorische of genetische stoornissen
- Kinderen die in het begin al medicijnen gebruiken, hoewel we weinig peuters vinden die al medicijnen gebruiken als ze met de behandeling beginnen. Als kinderen beginnen met medicatie nadat de behandeling is gestart, bewaren we alle informatie en gegevens met betrekking tot mogelijke medicatie-effecten en brengen we medicatiegebruik indien nodig statistisch over
- Comorbiditeit, deelnemer gediagnosticeerd met andere ziekten
- Familie/deelnemer niet in staat om op te volgen

10 1:1 sessies van een uur per week gedurende 10 weken met ouder en interventionist.
Strategieën voor gedragseducatie zullen worden gericht

De verzorger ontmoet 1 keer (1 uur per sessie) per week gedurende 10 weken met een interventionist - verzorger krijgt informatie over de ontwikkeling van het kind en krijgt de gelegenheid om vragen te stellen en de informatie met het eigen kind te bespreken

Actieve vergelijker: Door zorgverlener bemiddelde interventie
Een uur 1:1 met ouder, kind en interventionist, elke week, gedurende 10 weken zullen strategieën voor sociale communicatie en gezamenlijke betrokkenheid worden gericht

1 (twee 1/2 jour sessies per week) uur interventie per week gedurende 10 weken waarin ouders en hun kind de interventionist samen ontmoeten en de verzorger gebruiken als middel om verandering in de ontwikkeling van hun kind te faciliteren
