Multipolaire radiofrequente ablatie voor de behandeling van hepatocellulair carcinoom met behulp van klassieke intranodulaire techniek versus extra nodulaire techniek Zogenaamde "No Touch" -techniek: een prospectieve gerandomiseerde studie

Gedetailleerde beschrijving
206 patiënten met hepatocellulair carcinoom, waaronder maximaal drie knobbeltjes met een diameter tot vier cm, worden gerandomiseerd in twee therapeutische trajecten: multipolaire no-touch radiofrequentie versus multipolaire intranodulaire radiofrequentie. Patiënten die eerder zijn behandeld voor hepatocellulair carcinoom zullen niet in het onderzoek worden opgenomen. Diagnostiek van hepatocellulair carcinoom zal gebaseerd zijn op de richtlijn van de American Society of Liver Diseases. De vroege respons op de behandeling zal één maand na de radiofrequente ablatieprocedures worden beoordeeld (maximaal drie in geval van onvolledige necrose) met dynamisch contrastmiddel-versterkte CT- of MRI-leveronderzoeken. Voor de follow-up dynamische contrastmiddel-versterkte CT of MRI-leveronderzoeken zullen elke drie maanden worden uitgevoerd.
De proef duurt 73 maanden, waarvan 45 maanden voor de werving van patiënten. De belangrijkste beoordelingscriteria zijn het herhalingspercentage na 2 jaar.

Beschrijving
Inclusiecriteria:
- Volwassenen > 18 jaar, houder van maximaal 3 knobbeltjes met een diameter van minder dan 4 cm
- Diagnose van hepatocellulair carcinoom volgens niet-invasieve criteria van de American Society of Liver Study of op basis van histologisch bewijs
- Niet-invasieve diagnose van cirrose volgens de Franse Haute Authority of disease-richtlijn of op basis van histologisch bewijs
- Geen eerdere behandeling voor hepatocellulair carcinoom
- Multidisciplinaire behandelingsbeslissing door radiofrequente ablatie
Uitsluitingscriteria:
- Volwassen patiënt onder curatele of curatele, dakloos
- Patiënt met potentieel levensbedreigende ernstige co-infectie op korte termijn (afgezien van virale B- of C- of VIH-co-infectie)
- Zwangere vrouw of vrouw die borstvoeding geeft
- Patiënt voor wie regelmatige opvolging onmogelijk is, ongeacht de oorzaak
- Contra-indicatie voor algehele anesthesie
- Technische onmogelijkheid om de ingreep onder echogeleide uit te voeren
- Grens van de tumor op minder dan 1 cm afstand van de wand van de dikke darm of de hoofdgalwegen (hoofdkanaal rechts of links en hoofdkanaal)
- Tumor onzichtbaar met echografie
- Gebrek aan een veilige percutane cursus die kan worden gepland
- Tumor waarbij eerder meer dan vier biopsies zijn uitgevoerd (gecumuleerd tijdens een of meerdere eerdere biopsieën)
- Contra-indicatie om CT of MRI uit te voeren met contrastmiddel (GADOLINITE of jodaat) intraveneuze injectie
- Child-Pugh B- of C-cirrose (afgezien van de voorbijgaande leverfalen in de setting van acute hepatitis gerelateerd aan alcoholmisbruik)
- Totale loslating van het voorste deel van de lever van de inwendige buikwand als gevolg van overvloedige ascites.
- Protrombine-activiteit < 50 %
- Aantal bloedplaatjes <40 .10 3/ml
- Disfunctie van bloedplaatjes of aangeboren verminderde bloedstolling
