Gedetailleerde beschrijving
Internationale, multicenter, vergelijkende, open-label, parallelle groep, gemengde observatie-gerandomiseerde gecontroleerde studie.
Alle patiënten die kandidaat zijn voor de studie zullen volledige informatie ontvangen over de kenmerken, potentiële effectiviteit en bijwerkingen van de twee alternatieve behandelingen: radiotherapie (RT) en chirurgische behandeling. In aanmerking komende patiënten zullen worden gevraagd om gerandomiseerd te worden om behandeling A te krijgen (operatie, met of zonder RT) of behandeling B (definitieve RT) Wie randomisatie weigert, wordt opgenomen in de Prospective Cohort Study (PCS) en wordt naar keuze behandeld (behandelingsoptie A of behandelingsoptie B).
Hetzelfde radiotherapie- en chirurgische regime zal worden toegediend in het PCS en in het Randomized Clinical Trial (RCT) cohort

Beschrijving
Inclusiecriteria:
- Histologisch bevestigde diagnose (brachyury-expressie) van primair sacraal chordoom, van elke diameter en ontstaand op elke plaats van S1 tot coccyx.
- Leeftijd≥18 jaar
- ECOG-prestatiestatus (PS) 0-2
- Geen eerdere antineoplastische therapie
- Macroscopische tumor detecteerbaar op MRI/CT-scan
- Patiënt geschikt voor operatie
- Patiënt vatbaar voor RT
- Schriftelijke geïnformeerde toestemming gegeven vóór de inschrijving, volgens de internationale conferentie over harmonisatie/goede klinische praktijken (ICH/GCP).
Uitsluitingscriteria:
- Metastase op afstand
- Onvermogen om de behandelingspositie te behouden
- Voorafgaande radiotherapie aan het bekkengebied
- Voorafgaande therapie voor sacraal chordoom (inclusief chirurgie, cryoablatie, hyperthermie, enz.)
- Lokale omstandigheden die het risico op RT-toxiciteit verhogen (tumorverzweerde huidinfiltratie, niet-genezende infectie van zacht weefsel, fistel in behandelingsgebied)
- Rectale wandinfiltratie
- Algemene aandoeningen die het risico op RT-toxiciteit verhogen (actieve sclerodermie, xeroderma pigmentosum, cutane porfyrie)
- Aanwezigheid van een tweede actieve kanker (met uitzondering van niet-melanoom huidkanker in-situ cervixneoplasie en andere in-situ neoplasie)
- Ernstige comorbiditeit resulterend in een prognose van minder dan 6 maanden
- Onvermogen om geïnformeerde toestemming te geven
- Andere maligniteiten in de afgelopen 5 jaar
- Prestatiestatus ≥ 2 (ECOG).
- Aanzienlijke hart- en vaatziekten (bijvoorbeeld dyspnoe > 2 NYHA)
- Significante systemische ziekten graad >3 op de NCI-CTCAE v4.03-schaal, die de beschikbaarheid van de patiënt beperken of volgens het oordeel van de onderzoeker aanzienlijk kunnen bijdragen aan de behandelingstoxiciteit
- Vrouwen die zwanger zijn of borstvoeding geven
- Psychologische, familiale, sociale of geografische omstandigheden die het vermogen van de patiënt beperken om zich aan het protocol of de geïnformeerde toestemming te houden
