Prostaatkanker is de meest voorkomende niet-huidkanker bij mannen in de Verenigde Staten. Het doel van dit onderzoek is om te bepalen of dosis-geëscaleerde protonenbestralingstherapie een goede manier is om prostaatkanker met een hoog risico te behandelen.
Protontherapie (PT) is een soort ioniserende bestralingstherapie die de dosis overtollige straling die aan normale weefsels wordt afgegeven, vermindert. Door de stralingsdosis alleen te verhogen naar het gebied van de bekende tumor in de prostaat, zou men mogelijk de hoeveelheid overtollige straling die aan omliggende organen wordt afgegeven kunnen verminderen. toxiciteit.
In een poging om te profiteren van het potentieel van dosisescalatie om de ziektebestrijding te verbeteren, maar ook om de toxiciteit te beperken, is het gebruik van geavanceerde beeldvorming om prostaatkanker te identificeren en een focale stralingsboost aan het gebied te geven, nuttig gebleken. Recente ontwikkelingen in MRI hebben het tot de meest veelbelovende techniek gemaakt voor het identificeren en richten van IPT's, waardoor zowel de kankerbestrijdingspercentages als de toxiciteit worden verlaagd.
De studie kenmerkt hypofractionering en een gelijktijdige geïntegreerde boost voor de MRI-geïdentificeerde intraprostatische tumor (IPT) als een methode voor dosis-escalerende bestralingstherapie. De studie omvat patiënten met prostaatkanker met een hoog risico die het grootste risico lopen op recidief. Radiotherapie wordt gegeven in de loop van 8-9 weken. Bovendien zal androgeendeprivatietherapie (ADT) 8-10 weken voorafgaand aan de start van de bestraling worden gestart en in totaal 18 maanden worden voortgezet.

Inclusiecriteria:
- De patiënt moet studiespecifieke geïnformeerde toestemming geven op een IRB-goedgekeurde toestemming voorafgaand aan onderzoeksgerelateerde procedures of onderzoeksbehandelingen.
- Patiënt moet ten minste 18 jaar zijn op het moment van toestemming.
- Adenocarcinoom van de prostaat met AJCC Clinical Stage T1 tot T3b-ziekte met histologische evaluatie via biopsie of herhaalde biopsie binnen 12 maanden voorafgaand aan registratie.
- Patiënten moeten vóór de behandeling een diagnostische MRI van de prostaat ondergaan op een 1,5 T tot 3 T Tesla-machine binnen 6 maanden voorafgaand aan de onderzoeksregistratie.
- Een focale IPT moet zichtbaar zijn op MRI in de prostaat en/of zaadblaasjes en deze MRI moet binnen 6 maanden na het plannen van de CT-scan worden verkregen.
- Een biopsie van de dominante laesie wordt aanbevolen, maar is niet vereist. Als een echogeleide sextantbiopsie positief was voor prostaatadenocarcinoom in het gebied van de MRI-geïdentificeerde intraprostatische laesie, is dit acceptabel en is een andere geleide biopsie gericht op de MRI-geïdentificeerde ziekte niet nodig.
- Patiënten met ten minste één van de volgende risicofactoren: cT3a-T3b OF Gleason 9-10 OF PSA > 30 OF er moeten meer dan 1 risicofactor aanwezig zijn: klinisch stadium van T3, Gleason-score 8-10 of PSA 20 ng/ml of hoger.
- Hemoglobine moet ≥ 10 g/ml zijn binnen 4 maanden voorafgaand aan registratie.
- De prestatiestatus van Zubrod moet binnen 4 maanden voorafgaand aan de registratie 0-1 zijn.
- Als de patiënt in staat is om kinderen te krijgen, moet hij/zij bereid zijn om tijdens de behandeling medisch aanvaardbare anticonceptie te gebruiken en moet het advies worden gegeven om deze gedurende ten minste 1 jaar daarna te gebruiken. Dit is niet van toepassing als de patiënt niet seksueel actief is of een vasectomie heeft ondergaan.
- Patiënten moeten binnen 16 weken na aanmelding met de behandeling kunnen starten.
Uitsluitingscriteria:
- T4 prostaatziekte op CT, MRI of lichamelijk onderzoek.
- Patiënten die geen MRI van de prostaat kunnen ondergaan.
- Patiënten met een verandering van meer dan 25% in het prostaatvolume van de voorbehandelings-MRI van de prostaat die de IPT demonstreert en de MRI voor het plannen van de behandeling. Patiënten moeten in dit geval een herhaalde diagnostische MRI ondergaan op een 1,5T tot 3,0T Tesla-machine en er moet nog steeds een IPT zichtbaar zijn.
- IPT dat is meer dan 75% van het prostaatvolume gemeten op de CT-simulatiescan.
- Bewijs van metastase op afstand (M1).
- Patiënten met positieve klieren op cross-sectionele beeldvorming.
- Eerdere lokale behandeling van prostaatkanker, waaronder prostatectomie, hyperthermie, gefocusseerde echografie met hoge intensiteit, brachytherapie, uitwendige bestralingstherapie en/of cryotherapie.
- Eerdere bekkenbestralingstherapie.
- Geen eerder myocardinfarct in de afgelopen 6 maanden, ernstig congestief hartfalen of nierziekte in het eindstadium.
- Actieve inflammatoire darmziekte (diverticulitis, ziekte van Crohn, colitis ulcerosa) die het rectum aantast.
- Bilaterale heupprothese
Eerdere intrapelvische chirurgie. Dit omvat het volgende:
• Blaasoperatie
- Voorafgaande transurethrale resectie van de prostaat (TURP) of laserablatie voor goedaardige prostaathyperplasie (BPH).
- Patiënten die continue en huidige antistolling krijgen met warfarine-natrium (Coumadin), heparine-natrium, clopidogrelbisulfaat (Plavix), dabigatran etexilaatmesylaat (Pradaxa), rivaroxaban (Xarelto), apixaban (Eliquis), edoxaban (Savaysa), enoxaparine-natrium (Lovenox), prasugrel (Efient), ticagrelor (Brilinta), aspirine/er dipyridamol (Aggrenox) of natriumfondaparinux (Arixtra).

De prostaat en proximale zaadblaasjes worden behandeld tot 2 kobaltgrijs-equivalent per fractie voor 39 fracties voor een totaal van 78 kobaltgrijs-equivalent. Gelijktijdige geïntegreerde boost aan de IPT wordt geleverd tot 2,2 kobaltgrijs-equivalent per fractie voor 39 fracties voor een totaal van 85,8 kobaltgrijs-equivalent. De behandeling wordt eenmaal per dag gegeven, ongeveer 5 behandelingen per week (maandag-vrijdag), gedurende 8-9 weken.

De prostaat, proximale zaadblaasjes en bekkenknopen worden behandeld met 2 kobaltgrijs-equivalent per fractie voor 23 fracties voor een totaal van 46 kobaltgrijs-equivalent. De prostaat en de proximale zaadblaasjes worden behandeld met 2 extra kobaltgrijsequivalenten per fractie voor 16 fracties voor een totaal van 32 kobaltgrijsequivalenten. Behandel het gehele niet-betrokken zaadblaasje naar keuze met 2 kobaltgrijs equivalent per fractie gedurende 39 fracties voor een totaal van 78 kobaltgrijs equivalent wanneer een deel van het zaadblaasje is betrokken bij de tumor. Gelijktijdige geïntegreerde boost aan de IPT wordt geleverd tot 2,2 kobaltgrijs-equivalent per fractie voor 39 fracties voor een totaal van 85,8 kobaltgrijs-equivalent. De behandeling wordt eenmaal per dag gegeven, ongeveer 5 behandelingen per week (maandag-vrijdag), gedurende 8-9 weken.

Lokale persistentie wordt gedefinieerd als het falen van de oorspronkelijke abnormale tumormassa die te zien is op cross-sectionele beeldvorming en/of gevonden op digitaal rectaal onderzoek om te verdwijnen bij patiënten die ook een slechte PSA-respons hadden met een PSA-nadir van meer dan 1 ng/ml.
Lokaal recidief wordt gedefinieerd als de ontwikkeling van een nieuwe abnormale prostaatmassa op beeldvorming in dwarsdoorsnede of op digitaal rectaal onderzoek waarbij eerdere onderzoeken of beeldvorming geen massa vertoonden in het gebied voorafgaand aan bestralingstherapie of na de eerste behandeling.
