Infeksjon med alvorlig akutt respiratorisk syndrom coronavirus 2 (SARS-CoV-2) har vært et verdenshelseproblem i løpet av de siste månedene, og påvirker hovedsakelig land med høy metabolsk risiko, som Mexico.
Pasienter med type 2 diabetes (T2D) har økt risiko for enhver form for infeksjon samt økt dødelighetsrisiko.
Hyperglykemi har blitt etablert som en viktig prediktor for dødelighet hos pasienter med T2D og SARS-CoV-2.
Standardbehandlingen av hyperglykemi hos innlagte pasienter har vært basert på insulinskjemaer, men nylig tyder bevis på nytten av noen andre medikamenter, reduserer risikoen for hypoglykemi og øker sannsynligheten for en riktig metabolsk kontroll.
Målet med denne studien er å sammenligne nytten av dipeptidylpeptidase-4-hemmer (DPP4i) som en kombinasjon med insulin på metabolsk kontroll og prognose hos innlagte pasienter med SARS-CoV-2 og hyperglykemi.

Infeksjon med alvorlig akutt respiratorisk syndrom coronavirus 2 (SARS-CoV-2) har vært et verdenshelseproblem i løpet av de siste månedene, og påvirker hovedsakelig land med høy metabolsk risiko, som Mexico.
Pasienter med type 2 diabetes (T2D) har økt risiko for enhver form for infeksjon samt økt dødelighetsrisiko.
Hyperglykemi har blitt etablert som en viktig prediktor for dødelighet hos pasienter med T2D og SARS-CoV-2.
Standardbehandlingen av hyperglykemi hos innlagte pasienter har vært basert på insulinskjemaer, men nylig tyder bevis på nytten av noen andre medikamenter, reduserer risikoen for hypoglykemi og øker sannsynligheten for en riktig metabolsk kontroll.
DPP4-enzym har en allestedsnærværende distribusjon og har blitt betraktet som et pro-inflammatorisk enzym, med tanke på at DPP4-hemmere kan ha en anti-inflammatorisk effekt, som det har vist seg i forskjellige arbeider.
Målet med denne studien er å sammenligne nytten av DPP4-hemmer som en kombinasjon med insulin på metabolsk kontroll og prognose hos innlagte pasienter med SARS-CoV-2 og hyperglykemi.
For dette formålet vil vi randomisere pasienter med SARS-CoV-2 og hyperglykemi til å motta enten kombinasjonen av DPP4i + insulin eller insulin alene og vil følge algorithmebehandling for å definere metabolsk kontroll så vel som prognose.

Inklusjonskriterier:
- Innlagte pasienter med bekreftet infeksjon med SARS-CoV-2
- Hyperglykemi høyere enn 140 mg/dl
- Pasienter som aksepterer orale medisiner
- Både sex
- Eldre enn 18 år
- Pasienter som godtar å delta i studien og signerer samtykkeskjemaet
Ekskluderingskriterier:
- Type 1 diabetes
- Svangerskap
- Hyperosmolar hyperglykemisk tilstand eller diabetisk ketoacidose
