尽管结核病 (TB) 是人类已知的疾病已有 9000 多年的历史,但它仍然是发展中国家的主要公共卫生问题,这主要是由于所谓的高流行场所,例如监狱。
它是一种传染病,通过空气传播,结核病可以表现为活动型或潜伏型。 尽管传播具有生物学方面的意义,但不健康的环境条件(没有阳光直射的房间、通风不良和过度拥挤)和个体因素(营养不良、免疫抑制、使用酒精和其他药物)对可传播性和传染性有重大影响。
随着对结核分枝杆菌有活性的药物的发现,人们观察到世界上疾病发病率有所下降。 尽管发病率下降,世界卫生组织 (WHO) 仍将结核病归类为公共卫生问题,因为出现了多药菌株或对治疗产生广泛耐药性,加上随着艾滋病毒/艾滋病的出现观察到的潜伏性结核病再激活病例。
研究表明结核病的发病率与监狱环境之间存在直接关系。 然而,一个地方的监狱增加了这种疾病的发病率,表明监狱和社区之间存在疾病交换。
目前,监狱系统对结核病的控制是基于对活动性结核病和/或潜伏性结核病患者的追踪,以及他们各自的治疗情况。 对于活动性疾病个体的识别,有必要识别呼吸道症状(主要是咳嗽)和痰涂片镜检和痰培养,以及胸部 X 线检查。 作为寻找具有潜伏形式的个体,必须使用纯化蛋白衍生(PPD)进行皮肤测试。
巴西监狱系统的大多数单位都没有这些诊断方法,因此需要运送个人以实现这些方法,这会产生额外的成本,在大多数情况下,流程执行不可行。
随着这项研究的完成,它试图确定初级预防在监狱人口中的有效性,以便收集新的科学证据,为监狱中的结核病控制提供负担得起的方法。
尽管结核病的诊断和治疗取得了进展,但它仍是全球传染病死亡的第三大原因(Naghavi 等人,2015 年)。 2015年,世界卫生组织估计全球新发结核病病例为960万,死亡人数约为110万。 在巴西,估计发病率为每 100,000 人 44 例(WHO,2015)。 结核病的发病率每年下降约 2%,但这一比率在全球范围内并不均匀。
巴西在世界卫生组织的排名中排名第 22 位,估计年发病率为 83,310 例(Zumla 等人,2015 年)。 在过去七年中,估计发病率仅下降了 0.7%(每年)。 巴西和其他新兴国家结核病控制进展缓慢的一个关键因素是存在高风险亚人群,包括贫民窟和监狱,它们充当疾病传播的储存库和放大器(Basu;Stückler;Mckee 2011) ; Dowdy 等人,2012 年)。 最近的一项系统审查表明,监狱人口中结核病的平均发病率可能高达普通人群的 23 倍(Baussano 等人,2010 年)。
巴西是世界第四大监狱人口,在过去七年中,囚犯中的结核病发病率有所上升。 尽管囚犯仅占人口的 0.3%,但在此期间监狱人口的增加导致囚犯中发生的所有结核病病例的比例几乎翻了一番(2007 年为 4.1%,2013 年为 8%,2013 年为 1%)。
逮捕是结核病传播的理想环境,因为他们显示个人吸食高剂量的烟草和酒精,此外还有人在拥挤的牢房里滥用药物,通风不良,获得保健和诊断结核病的机会有限。 目前,卫生部建议在监狱入口处积极寻找结核病患者,并每年进行一次胸部X光检查。 出于成本和后勤方面的考虑,大多数监狱不遵守此建议。 还有一个明确的建议是不要使用结核菌素皮肤试验或进行潜伏性结核病的治疗。 主动病例检测和/或预防性治疗的程序是否会影响监狱中的高传播率仍然是一个悬而未决的问题(Al-Darraji;Kamarulzamn;Altice,2012 年)。
监狱中结核病病例的集中既可能是控制疾病的障碍,也可能是控制疾病的机会,这取决于在这些环境中干预措施的有效性。
