Inkrementální a decimentní titrace PEEP různými strategiemi u subjektů s ARDS: prospektivní fyziologická studie (ARDS,PEEP)

Výzkum přesných individualizovaných mechanických ventilačních strategií pro ARDS vedený monitorováním a hodnocení rozdílů ve ventilaci a distribuci perfúze s elektrickou impedancí
Vyšetřovatelé provedli prospektivní fyziologickou pozorovací studii na populaci ARDS, popsali jejich fyziologické a EIT indexy s různými metodami titrace PEEP a také porovnali tyto indexy, aby určili, zda existují rozdíly mezi různými metodami titrace PEEP. Prozkoumejte a odhalte fyziologické rozdíly v reakci na optimální PEEP různými způsoby , a analyzovali korelaci optimálního PEEP těmito různými způsoby.

Popis
Kritéria pro zařazení:
- Intubovaní a mechanicky větrané pacienty na JIP do 48 hodin
- Být starší 18 let
- S mírným severem ARDS (PAO2 /FIO2, 200 mm Hg)
Kritéria pro vyloučení:
- Těžká chronická obstrukční plicní onemocnění
- Plicní bulla (> 1 cm) nebo pneumotorax nebo pneumomediastinum nebo subkutánní emfyzém
- Těhotenství
- BMI≥35 kg/m2
- Podezření na plicní vaskulární krvácení
- Narušení vědomí nebo intrakraniální hypertenze
- Těžká srdeční dysfunkce (New York Heart Association Class III nebo IV, akutní -koronární syndrom nebo přetrvávající komorová tachyarhytmie), kardiogenní šok nebo po velké srdeční chirurgii
- Hemodynamická nestabilita (tj. Průměrný arteriální tlak [mapa] <60 mm Hg navzdory použití vasopresoru
- Lékařský tým zvažuje krátká období vysokého tlaku jako nebezpečné
- Kontraindikace pro umístění tlakových katétrů jícnu (např. Ezofageální varix nebo zlomeninu lebky)
- Kontraindikace pro monitorování EIT (včetně kardiostimulátoru nebo kožních onemocnění, které omezují umístění elektrod)
- Agonální fáze nebo konečné stadia nemocí

SpO2/FiO2 measured at each selected PEEP.
This study will separately measure peripheral oxygen saturation (SpO₂, unit: %) and fraction of inspired oxygen (FiO₂, unit: %).
Because both measurements share the same unit (%), they will not be reported as separate outcome measures.
Instead, they will be aggregated into a single reported value: the SpO₂/FiO₂ ratio (dimensionless).
At each selected positive end-expiratory pressure (PEEP) level, the ratio will be calculated as S/F = SpO₂ (%) ÷ FiO₂ (%), and this ratio will be recorded as the final outcome measure.
