Effetti fisiologici e psicologici del testosterone durante grave deficit energetico e recupero: uno studio randomizzato controllato con placebo
L'obiettivo di questo studio è determinare se il mantenimento di uno stato eugonadico, durante un deficit energetico grave e sostenuto, attenua i decrementi fisiologici, in particolare la perdita di massa magra.

Questo studio arruolerà fino a 60 uomini fisicamente attivi in uno studio randomizzato in 3 fasi, controllato con placebo.
Dopo aver completato una fase di 14 giorni (vita libera, fase 1), energia adeguata, fase di acclimatazione alla dieta (proteine, 1,6 g∙kg-1∙d-1; apporto energetico totale di grassi 30%, con l'energia rimanente derivata dai carboidrati ), i partecipanti saranno randomizzati in uno dei due gruppi sperimentali e sottoposti a una fase di deficit energetico di 28 giorni (live-in, fase 2), 55%: solo deficit energetico (DEF) o deficit energetico + testosterone esogeno (DEF+TEST) .
Il recupero (vita libera, fase 3) sarà valutato dopo aver completato la fase 2 per determinare quando la massa corporea è stata recuperata entro ± 2,5% della massa corporea iniziale (la durata può variare, massimo 42 giorni per la fase 3).
Questo studio delineerà il contributo del declino del testosterone dalle esigenze fisiche e mentali incontrate dai combattenti durante l'addestramento militare e le operazioni di combattimento su marcatori complessi dello stato fisiologico e psicologico, affrontando una lacuna diretta, costantemente osservata, nella conoscenza.

Criterio di inclusione:
- Fisicamente attivo (almeno 2 giorni alla settimana esercizio aerobico e/o di resistenza)
- Non assumere alcun farmaco su prescrizione e/o disposto ad astenersi da qualsiasi uso di farmaci prima e durante l'intero periodo di studio, a meno che non sia fornito/approvato dal medico dello studio
Disponibilità ad astenersi dall'alcol, dal fumo, dalle sigarette elettroniche o dall'uso di qualsiasi prodotto a base di nicotina, caffeina e integratori alimentari durante l'intero periodo di studio
- A discrezione del medico dello studio, il periodo di sospensione per farmaci, integratori e farmaci da banco è ≥ da 1 a 4 settimane
- Il periodo di wash-out per caffeina e alcol è ≥ 7 giorni
- Disposto a vivere nell'unità di degenza del Pennington Biomedical Research Center per 28 giorni consecutivi
- Disposto a sottoporsi a uno screening antidroga sulle urine
- Soddisfa gli standard di composizione corporea dell'esercito degli Stati Uniti specifici per età secondo il regolamento dell'esercito 600-9, che include stime della percentuale di grasso corporeo in base a misure di altezza, peso e circonferenza (collo e vita)
- La concentrazione totale di testosterone rientra nel normale intervallo fisiologico, testosterone totale (300-1.000 ng/dL).
Criteri di esclusione:
- Lesioni muscoloscheletriche che compromettono la capacità di esercizio
- Disturbi cardiometabolici diagnosticati (ad es. ipertensione, iperlipidemia, malattie renali, diabete, ecc.)
- Allergie o intolleranze alimentari, pratiche vegetariane o anamnesi di complicazioni con la lidocaina
- Uso di steroidi anabolizzanti, ormone della crescita umano o integratori alimentari simili a precursori del testosterone negli ultimi 6 mesi
- Non si asterrà dal fumare (qualsiasi prodotto a base di nicotina), alcol, caffeina o qualsiasi altro integratore alimentare durante lo studio
- - Qualsiasi uso di antibiotici, ad eccezione degli antibiotici topici, entro 3 mesi dalla partecipazione allo studio
- Colonscopia entro 3 mesi dalla partecipazione allo studio
- Uso cronico di lassativi, emollienti delle feci, antiacidi o farmaci antidiarroici (≥ una volta alla settimana)
- Anamnesi di malattia gastrointestinale (ad es. celiachia, sindrome dell'intestino irritabile, colite, morbo di Crohn)
- Mangiatore trattenuto (il questionario sull'alimentazione a tre fattori) come valutato dallo staff di valutazione psicologica e comportamentale dello studio
- Adulti incapaci di acconsentire

- Protesi metalliche, claustrofobia, dimensioni della testa incompatibili con le apparecchiature per la risonanza magnetica, ecc.
- Sedentario o impegnato in meno di 2 giorni di attività fisica a settimana (allenamento aerobico e/o di resistenza)
- Supera gli standard di composizione corporea dell'esercito americano specifici per età secondo il regolamento dell'esercito 600-9
- Storia precedente di calcoli renali se non diversamente approvato dall'investigatore medico
- Pressione arteriosa sistolica > 150 o pressione arteriosa diastolica > 95 mmHg
- Precedente storia di cancro al seno o alla prostata
- Storia precedente di broncopneumopatia cronica ostruttiva o apnea notturna ostruttiva
- Risultati dei risultati di laboratorio dell'antigene prostatico specifico > 3 ng/ml, ematocrito > 50% o screening farmacologico positivo nelle urine

I partecipanti assegnati in modo casuale alla condizione di controllo saranno soggetti a dispendio energetico indotto dall'esercizio con conseguente deficit energetico del 55% e verranno somministrate iniezioni di placebo di olio di sesamo durante la fase 2 della sperimentazione.
