Fysiologische en psychologische effecten van testosteron tijdens ernstig energietekort en herstel: een gerandomiseerde, placebo-gecontroleerde studie
Het doel van deze studie is om te bepalen of het handhaven van een eugonadale toestand, tijdens een ernstig, aanhoudend energietekort, fysiologische achteruitgang verzacht, met name het verlies van vetvrije massa.

Deze studie zal maximaal 60 fysiek actieve mannen inschrijven in een 3-fase gerandomiseerde, placebo-gecontroleerde studie.
Na het voltooien van een 14-daagse (vrij leven, fase 1), energie-adequate, dieet-acclimatisatiefase (eiwit, 1,6 g∙kg-1∙d-1; vet 30% totale energie-inname, met resterende energie afkomstig van koolhydraten ), worden de deelnemers gerandomiseerd in een van de twee experimentele groepen en ondergaan ze een fase van 28 dagen (live-in, fase 2), 55% energietekort: alleen energietekort (DEF) of energietekort + exogeen testosteron (DEF+TEST) .
Herstel (vrij leven, fase 3) wordt beoordeeld na het voltooien van fase 2 om te bepalen wanneer het lichaamsgewicht is hersteld binnen ± 2,5% van het oorspronkelijke lichaamsgewicht (duur kan variëren, maximaal 42 dagen voor fase 3).
Deze studie zal de bijdrage van testosterondalingen afbakenen van de fysieke en mentale eisen die Warfighters tegenkomen tijdens militaire training en gevechtsoperaties op complexe markers van fysiologische en psychologische status, waarbij een directe, consistent waargenomen leemte in kennis wordt aangepakt.

Inclusiecriteria:
- Lichamelijk actief (minstens 2 dagen per week aërobe en/of weerstandsoefeningen)
- Geen voorgeschreven medicijnen nemen en/of bereid zijn om voorafgaand aan en gedurende de gehele onderzoeksperiode af te zien van elk medicijngebruik, tenzij verstrekt/goedgekeurd door de onderzoeksarts
Bereid om gedurende de gehele onderzoeksperiode af te zien van alcohol, roken, e-sigaretten of het gebruik van nicotineproducten, cafeïne en voedingssupplementen
- Naar goeddunken van de onderzoeksarts is de wash-outperiode voor medicijnen, supplementen en vrij verkrijgbare medicijnen ≥ 1 tot 4 weken
- De uitwasperiode voor cafeïne en alcohol is ≥ 7 dagen
- Bereid om 28 opeenvolgende dagen op de klinische afdeling van het Pennington Biomedical Research Center te wonen
- Bereid om een urinedrugscreening te ondergaan
- Voldoet aan leeftijdsspecifieke normen voor lichaamssamenstelling van het Amerikaanse leger volgens Army Regulation 600-9, die schattingen bevat van het percentage lichaamsvet op basis van lengte, gewicht en omtrekmetingen (nek en taille)
- De totale testosteronconcentratie ligt binnen het normale fysiologische bereik, het totale testosteron (300-1.000 ng/dL).
Uitsluitingscriteria:
- Musculoskeletale letsels die het inspanningsvermogen in gevaar brengen
- Gediagnosticeerde cardiometabolische stoornissen (d.w.z. hypertensie, hyperlipidemie, nierziekte, diabetes, enz.)
- Allergieën of intolerantie voor voedsel, vegetarische praktijken of voorgeschiedenis van complicaties met lidocaïne
- Gebruik van anabole steroïden, menselijk groeihormoon of testosteronprecursorachtige voedingssupplementen in de afgelopen 6 maanden
- Zal tijdens het onderzoek niet afzien van roken (enig nicotineproduct), alcohol, cafeïne of enig ander voedingssupplement
- Elk gebruik van antibiotica, behalve lokale antibiotica, binnen 3 maanden na deelname aan de studie
- Colonoscopie binnen 3 maanden na studiedeelname
- Chronisch gebruik van laxeermiddelen, middelen die de ontlasting verzachten, antacida of middelen tegen diarree (≥ eenmaal per week)
- Geschiedenis van gastro-intestinale aandoeningen (bijv. coeliakie, prikkelbaredarmsyndroom, colitis, ziekte van Crohn)
- Beperkte eter (de Three-Factor Eating Questionnaire) zoals beoordeeld door de psychologische en gedragsbeoordelingsmedewerkers van de studie
- Volwassenen kunnen niet instemmen
- Vrouwen
- Gevangenen
- Metalen implantaten, claustrofobie, hoofdomvang die niet compatibel is met MRI-apparatuur, enz.
- Sedentair of houdt zich bezig met <2 dagen lichamelijke activiteit per week (aerobe training en/of weerstandstraining)
- Overtreft de leeftijdsspecifieke normen voor lichaamssamenstelling van het Amerikaanse leger volgens Army Regulation 600-9
- Voorgeschiedenis van nierstenen, tenzij anders goedgekeurd door de medisch onderzoeker
- Systolische bloeddruk > 150 of diastolische bloeddruk > 95 mmHg
- Voorgeschiedenis van borst- of prostaatkanker
- Voorgeschiedenis van chronische obstructieve longziekte of obstructieve slaapapneu
- Bevindingen van laboratoriumresultaten van prostaatspecifiek antigeen > 3 ng/ml, hematocriet > 50% of positieve urinedrugscreening
- Op basis van het klinische oordeel van het onderzoeksteam is het mogelijk dat een proefpersoon niet geschikt is voor deelname aan het onderzoek

Deelnemers die willekeurig worden toegewezen aan de controleconditie zullen worden onderworpen aan door inspanning geïnduceerd energieverbruik, resulterend in een energietekort van 55% en zullen placebo-injecties met sesamolie krijgen tijdens fase 2 van het onderzoek.

Experimenteel: Energietekort + testosteron
Deelnemers die willekeurig worden toegewezen aan de interventieconditie zullen worden onderworpen aan door inspanning geïnduceerd energieverbruik, resulterend in een energietekort van 55%, en zullen testosteron-enanthaat-injecties toegediend krijgen tijdens fase 2 van het onderzoek.
