股関節骨折の高齢患者は、栄養失調の割合が高いです。
70 歳以上の股関節骨折患者のコホート研究では、18.8% が栄養失調であり、44.6% が栄養失調のリスクがあります。
増加する証拠は、術前の栄養状態が手術後の高齢患者の臨床転帰に重大な影響を与えることを示唆しています。
たとえば、術前の栄養失調または栄養失調のリスクは、せん妄の増加、せん妄以外の合併症の増加、入院期間の延長、死亡率の高さ、および手術後の長期生存率の悪化と関連しています。
周術期の栄養介入は、高齢の股関節骨折患者の術後のせん妄を減らし、早期および長期の臨床転帰を改善する可能性があると推測しています。

- 年齢 70 歳以上;
- 股関節骨折で入院、48時間以内に股関節骨折手術予定。
- 簡易型ミニ栄養評価(MNA-SF)に従って、栄養失調または栄養失調のリスクがあると分類されている;
- 書面によるインフォームドコンセントを提供します。
除外基準:
- 病的骨折;
- -統合失調症、てんかん、パーキンソン病、または手術前の重症筋無力症の病歴;
- 昏睡、重度の認知症、または言語の壁のためにコミュニケーションが取れない;
- 胃腸系の病気のために食べることができません;
- 重度の肝機能障害 (Child-Pugh クラス C)、腎不全 (腎代替療法の必要性)、呼吸不全 (呼吸補助の必要性)、心不全 (ニューヨーク心臓協会分類 ≥IV)、または米国麻酔科学会分類 ≥IV ;
- -手術前の既存の臓器損傷(せん妄、急性腎損傷、心筋損傷など);
