適格なインフォームド コンセントが得られた場合、患者は次の 3 つのアームのいずれかに 1:1:1 で無作為に割り付けられます。
- 標準治療 (SoC) 結核治療 + 結核治療の最初の 4 週間は 1 日 2 回プラセボ、その後 4 週間は 1 日 1 回プラセボ。 (対照群)。
- SoC TB 治療 + アセチルサリチル酸 300mg を 1 日 2 回 TB 治療の最初の 4 週間、続いてアセチルサリチル酸 300mg を 1 日 1 回、さらに 4 週間。
- SoC TB 治療 + 結核治療の最初の 4 週間はイブプロフェン 400mg を 1 日 2 回、その後さらに 4 週間はイブプロフェン 400mg を 1 日 1 回投与します。

説明
包含基準:
- 成人、18~60歳

- -WHOが推奨するアッセイによる結核菌(Mtb)の検出を報告する喀痰検査結果のハードコピーとして定義される検査で確認された肺結核(肺外病変の有無にかかわらず) - 迅速な分子アッセイまたはその後の種分化を伴うマイコバクテリア培養の両方が包含基準。
- 妊娠可能な女性(女性を含む)
- 参加者は、無作為化の時点で患者が HIV に感染しているという説得力のある証拠がない限り、HIV 検査を喜んで受けなければなりません。
除外基準:
- アスピリン、イブプロフェン、またはその他の NSAID による治療が必要な併存疾患がある (例: 循環器疾患、リウマチ熱、慢性疼痛など)

- -無作為化前の6か月で3日以上の多剤結核治療(リファマイシンとイソニアジド予防治療レジメンを含む)の受領。 結核治療開始の前月に 3 日以上の結核予防治療を受けた参加者も除外されます。
- 現在妊娠中・授乳中です。妊娠し、試験の最初の 4 週間で妊娠していることが判明した女性は、試験から除外され、分析から除外されます。
-無作為化の前に取得された次の検査パラメータのいずれか:
- -アスパラギン酸アミノトランスフェラーゼ(AST)またはアラニンアミノトランスフェラーゼ(ALT)>正常上限の3倍(ULN);
- 総ビリルビン > 2 x ULN;
- -好中球数≤700好中球/mm3;
- 血小板数 < 50,000 細胞 / mm3

無作為化前の3か月間の同時治療、または試験の過程で計画された治療は、次の薬剤のいずれかでフォローアップします:
- 抗凝固療法
- 免疫調節療法(がん治療、吸入ステロイドの経口または毎日の使用;
- 制酸剤またはプロトンポンプ阻害剤 - 自己治療および処方を含む
- NSAID に起因する過敏症、喘息またはアレルギーの病歴または臨床記録
- ベースライン時の体重が45kg未満。
過去2年間に以下のいずれかを示唆する病歴または臨床記録:
- 消化性潰瘍疾患または胃腸出血、
- 凝固障害またはその他の出血性疾患、
- -入院を必要とする腎疾患 - さらに、急性腎障害の任意の時点での以前の記録は除外基準になります。
- さらなる調査または入院が必要な肝疾患、
- 根底にある心血管疾患または心血管疾患の危険因子。
-HIV感染患者(ARTステータスに関係なく):
- CD4
- ARTの場合、抑制されていない(> 200コピー/ ml)ウイルス量
- ART を受けていない場合、主治医の意見または地域の ART ガイドラインに従って、患者は 8 週間の初期プラセボまたは NSAID 治療段階で ART を開始する必要があります。
- アルコール使用:潜在的な参加者の自己報告または研究者の意見で、患者は通常の1週間で1日あたり平均4単位以上飲むか、大量飲酒者(男性:5杯以上の飲酒、女性:4杯以上の飲酒)約2時間で飲む)。

Time taken to reach 67% sustained reduction in the TB score over the course of TB treatment.
Minimum value of the score = 0; Maximum value = 13).
Higher scores mean a better or worse outcome.
Difference between each intervention arm and control group were analysed.
The TB Score has been described to be useful to monitor good response to TB treatment, regardless of HIV status.

Improvement or Resolution of Clinical Signs and Symptoms

Signs and symptoms were assessed using the TB Score (TBS), which ranges from 0 to 13, with higher scores indicating more severe disease.
The outcome was defined as the proportion of participants achieving a ≥50% reduction from baseline in TBS at Week 8 and a ≥75% reduction from baseline in TBS at Week 24.
Comparisons were performed between each intervention arm and the control group.
TBS has been described as a useful tool for monitoring clinical response to tuberculosis treatment.

Changes in chest X-ray (CXR) findings were assessed using the Activity component of the RUTI TB Evolution Score (RTBES). The baseline CXR served as the reference for comparison with subsequent CXRs obtained during tuberculosis treatment. The Activity component ranges from 0 to 38, with higher scores indicating more severe radiographic involvement. The outcome was defined as the proportion of participants achieving a ≥50% reduction from baseline in the Activity component at Week 8 and a ≥75% reduction from baseline at Week 24. Comparisons were performed between each intervention arm and the control group. |
