De patiëntactiviteiten van het dagelijks leven voor onderzoek naar amyotrofische laterale sclerose (PADL-ALS, bijlage B) is een patiëntgerichte herziening van de standaard herziene functionele beoordelingsschaal voor ALS die wordt gebruikt in klinische onderzoeken, de ALSFRS-R. De PADL-ALS is ontwikkeld op basis van interviews met patiënten en focusgroepen van patiënten. De ALSFRS-R bestaat uit 12 categorieën die verschillende activiteiten van het dagelijks leven beschrijven en omvat zes bulbaire ademhalingsfuncties, drie functies van de bovenste ledematen (schrijven, voedsel snijden en aankleden) en drie grove motorische functies (lopen, klimmen en draaien). in bed). Elke activiteit wordt geregistreerd naar de beste benadering uit een lijst van vijf keuzes, met een score van 0-4, met een totale score variërend van 48 (normale functie) tot 0 (geen functie). De PADL-ALS bevat vragen uit de ALSFRS-R, met herzieningen om de vragen begrijpelijker te maken. Daarnaast bevat de PADL-ALS een vraag over pijn; een vraag over emotionele labiliteit; en een algemene niet-confessionele vraag over geloof. Het onderzoek bestaat uit twee delen, het eerste onderzoek en daarna maandelijkse vervolgonderzoeken. Het eerste onderzoek omvat de PADL-ALS met aanvullende vragen over het begin van de symptomen, de datum van de diagnose, de eerste betrokken regio, de indruk van de patiënt van de diagnose, algemene demografische vragen (leeftijd, geslacht, ras, etniciteit, opleiding) en medicijnen die verband houden met hun diagnose van motorneuronziekte.
De onderzoeksgegevens zullen worden opgeslagen door het Data Management and Coordinating Center (DMCC) van het Rare Diseases Clinical Research Network aan de Universiteit van Zuid-Florida. Na afloop van de studieperiode worden de gegevens verzonden naar Jeffrey Statland, MD, University of Kansas Medical Center (studievoorzitter) en Michael Benatar, MD, PhD, University of Miami (CReATe Consortium PI). Alle verzamelde gegevens worden naar de database van Genotypes en Fenotypes (dbGaP) gestuurd om voor onbepaalde tijd te worden opgeslagen.

- Zelfgerapporteerde diagnose van motorneuronziekte (amyotrofische laterale sclerose, amyotrofische laterale sclerose - frontotemporale dementie, primaire laterale sclerose, progressieve spieratrofie of erfelijke spastische paraplegie)
Ingeschreven bij CreATe Connect, een RDCRN-contactregister
o Individuen van elke leeftijd, ras, etniciteit en van elke locatie kunnen deelnemen
- Deelnemers moeten de enquête in het Engels kunnen invullen
