Evaluatie van darmbeeldvorming en ontlastingsbiomarkers bij patiënten met sclerodermie-geassocieerde gastro-intestinale aandoeningen (Pre Med SSc GI)
Precisiegeneeskunde bij systemische sclerose Gastro-intestinale aandoeningen: evaluatie van beeldvorming en ontlastingsbiomarkers voor het differentiëren van ziektestadia en behandelingsreacties
Systemische sclerose (SSc) wordt gekenmerkt door auto-immuniteit en vasculopathie resulterend in fibrose van de huid en inwendige organen, waaronder het maagdarmkanaal (GI). De belangrijkste onvervulde klinische behoeften zijn de beschikbaarheid van niet-invasieve biomarkers voor vroege diagnose van SSc-GI, verdere karakterisering van verschillende stadia van SSc-GI en SSc-GI behandelingsrespons. De onderzoekers stellen voor om MRI FDG-PET te combineren met MRI T1-MOLLI-mapping, die is toegepast op cardiale beeldvorming om histologisch gecorreleerde cardiale fibrose te kwantificeren. T1-MOLLI maakt detectie en kwantificering van diffuse fibrose mogelijk zonder dat er contrastmiddel nodig is.
Doel 1: FDG-PET-MRI-beeldvorming (primaire biomarker) en ontlastingsmarkers (secundaire biomarker) zullen worden vergeleken tussen patiënten met VEDOSS/vroege SSc en degenen met late SSc die geen immunosuppressieve behandeling ondergaan.
Doel 2: Evaluatie van de verandering in biomarkerniveaus vanaf de baseline vóór de behandeling tot 6 maanden (primair eindpunt) en 12 maanden (secundair eindpunt) na MMF-behandeling, bij vroege SSc-patiënten
Door gebruik te maken van precisiegeneeskunde bij diagnose en behandelingsevaluatie, verwachten de onderzoekers dat deze studie aanzienlijk zal bijdragen aan het bevorderen van managementstrategieën voor en het verbeteren van de resultaten van SSc-GI-ziekte.

Doel 1 Vaststellen of FDG-PET-MRI-beeldvormingsbiomarkers patiënten met VEDOSS/vroege SSc (voornamelijk inflammatoir) onderscheiden van patiënten met late SSc (voornamelijk fibrose). Ontlastingsmarkers zullen worden gebruikt als secundaire biomarkers die ontstekingen ondersteunen.
Onderzoeksopzet: dwarsdoorsnede; De onderzoekers zullen biomarkers vergelijken tussen patiënten met VEDOSS/vroege SSc en patiënten met late SSc die geen immunosuppressieve behandeling ondergaan.
Doel 2 Evalueer FDG-PET-MRI beeldvorming biomarker verandering gedurende een behandelingsperiode van 6 en 12 maanden met mycofenolaatmofetil (MMF) bij patiënten met vroege SSc. Ontlastingsmarkers zullen worden gebruikt als secundaire biomarkers die ontstekingen ondersteunen.

Inclusiecriteria:
(i) ≥ 21 jaar oud (ii) SSc die voldoet aan de criteria van het American College of Rheumatology/European League Against Rheumatism (EULAR) 2013 of VEDOSS die voldoet aan de door EULAR voorgestelde criteria
Doel 1 vakstratificatie:
(i) VEDOSS/ vroege SSc (≤3 jaar) of late SSc (> 5 jaar), met ziekteduur gedefinieerd vanaf het begin van het eerste niet-Raynaud-symptoom (ii) Geen immunosuppressieve behandeling of prednisolon >10 mg/dag 8 weken vóór de aanwerving
Doel 2 onderwerpstratificatie:
(i) vroege SSc (≤3 jaar) en (ii) ook starten met immunosuppressieve behandeling
- MMF + Prednisolon of
- Andere immunosuppressieve behandeling in combinatie met MMF + Prednisolon
Verkennend doel onderwerpstratificatie:
(i) VEDOSS of vroege SSc (≤3 jaar) met ziekteduur gedefinieerd vanaf het begin van het eerste niet-Raynaud-symptoom (ii) Geen immunosuppressieve behandeling of prednisolon >10 mg/dag 8 weken voor rekrutering
Uitsluitingscriteria:
(i) Borstvoeding of zwangerschap (ii) Allergie of contra-indicaties voor hyoscinebutylbromide (bijv. myasthenia gravis, prostaatvergroting met urineretentie, klinisch significante gastro-intestinale obstructie of ileus) (iii) Contra-indicaties voor MRI (iv) Infecties 4 weken vóór basislijnmetingen (v) Op antibiotica 4 weken vóór basislijnmetingen, tenzij gegeven voor behandeling van dunne darmbacteriën overgroei (SIBO), een complicatie van SSc.
(vi) Maligniteit of vermoedelijke maligniteit in de afgelopen 2 jaar (vii) Diabetes onder behandeling

Doel 1
De onderzoekers zullen PET-MRI-beeldvorming en ontlastingsbiomarkers vergelijken tussen patiënten met VEDOSS/vroege SSc (n=40) en degenen met late SSc (n=20) die geen immunosuppressieve behandeling ondergaan. Deelnemers ondergaan bij aanvang eenmalig een PET-MRI-scan. |
Deelnemers worden centraal gescand in het Clinical Imaging Research Center (CIRC, Singapore), op een Biograph mMR PET-MR-scanner.
Gecombineerde FDG-PET-MRI-scan is van cruciaal belang voor co-registratie van peristaltische darm voor optimale beeldkwaliteit.
De slokdarm naar het anorectum wordt in beeld gebracht.
De PET-MRI-scan begint 60 minuten na de FDG-injectie van 6 mCi en onmiddellijk na het injecteren van 10 mg hyoscinebutylbromide om de peristaltiek te verminderen.
MRI-sequenties zijn niet-contrast.

Bij vroege SSc-patiënten (n=35) zullen de onderzoekers de verandering in biomarkerniveaus bepalen vanaf de basislijn vóór de behandeling tot 6 maanden (primair eindpunt) en 12 maanden (secundair eindpunt) na MMF-behandeling. Deelnemers ondergaan PET-MRI-scans bij aanvang, 6 maanden en 12 maanden. |
