Diettintervensjon for bipolar lidelse
Målrettede endringer i n-3 og n-6 fettsyrer for håndtering av humørvariasjoner i vedlikeholdsfasen av bipolar lidelse

Etterforskerne vil gjennomføre en 2-arms, parallell gruppe, randomisert, kontrollert 24-ukers diettintervensjon for å evaluere den terapeutiske effekten av to diettintervensjoner hos pasienter med BD. Etter en to ukers baseline-overvåkingsperiode vil etterforskerne randomisere 84 pasienter med kronisk BD for å forsterke vanlig behandling med enten en eksperimentelt endret omega-3, omega-6 intervensjon eller en kontrolldiett med gjennomsnittlig amerikansk inntak på n-3 og n-6 fettsyrer. Forsøkspersoner i begge grupper vil forbli under behandling av sin lege under hele prøveperioden. I løpet av den første fasen av forsøket (12 uker) vil intervensjonen være intens, i den andre fasen (12 uker) vil en mindre intens intervensjon bli levert, og etter de to fasene av intervensjonen vil det være en 24-ukers oppfølging -opp periode.
Studien er designet for å oppnå følgende spesifikke mål og få støtte for følgende hypoteser:
Spesifikt mål 1 (primært humørutfall): Å sammenligne effektiviteten av den eksperimentelle diettintervensjonen med kontrolldietten for å redusere variasjonen i humørsymptomer og redusere generell psykologisk plage.
Hypotese 1: Sammenlignet med kontrolldietten vil den eksperimentelle intervensjonen gi betydelig forbedring i (1a) variasjon av daglige humørsymptomer, energi og impulsivitet ved bruk av en visuell analog skala målt ved bruk av økologisk momentan analyse (1); og (1b) psykiske plager og generell funksjon ved bruk av PROMIS-29.
Spesifikt mål 2 (sekundært humørutfall): Å sammenligne effektiviteten av den eksperimentelle diettintervensjonen med kontrolldietten for å redusere tilbakefall.
Hypotese 2: Sammenlignet med kontrolldietten vil den eksperimentelle intervensjonen gi betydelig reduksjon i tilbakefall av stemningsepisoder over en 12-måneders periode. For å gi tilstrekkelig kraft til denne studien for måling av tilbakefall, ville etterforskerne trenge en prøvestørrelse 2-3 ganger hva etterforskerne estimerer for denne studien, derfor er det primære humørutfallet ikke tilbakefall av episoden, men er måling av humørvariabilitet, som igjen spår gjentakelse. Etterforskerne vil måle tilbakefall for å samle data for fremtidige studier.
Spesifikt mål 3 (primært biokjemisk utfall): Å evaluere om den eksperimentelle diettintervensjonen kan endre n-6 AA og dets bioaktive metabolitter, n-3 DHA og dets bioaktive metabolitter hos pasienter med BD.
Hypotese 3: Sammenlignet med kontrolldietten vil den eksperimentelle intervensjonen signifikant (1a) endre n-6 AA og n-3 DHA i erytrocytter; og (1b) endre 17-hydroksy DHA og redusere PGE2 i plasma.
Undersøkende mål: På en utforskende måte vil etterforskerne også (1) sammenligne effekten av den eksperimentelle diettintervensjonen med kontrolldietten for å forbedre hodepinerelaterte kliniske endepunkter i undergruppen av BP-pasienter med komorbid migrene; (2) teste effekten av kosttilskuddene på et bredt utvalg av bioaktive derivater av n-3 og n-6 fettsyrer og andre inflammatoriske mediatorer (f.eks. cytokiner, CRP); og (3) evaluere om biokjemiske endringer i n-3 DHA, n-6 AA og deres bioaktive metabolitter er relatert til kliniske forbedringer (4) sammenligne effektiviteten av den eksperimentelle diettintervensjonen med kontrolldietten for å forhindre tilbakefall av sykdom i etterkant- opp; (5) lagre prøver for utforskende biomarkøranalyse.

- Historie med spesifikke matallergier som, men ikke begrenset til, fisk, gluten, meieriprodukter
- Sterk aversjon mot fisk
- Enhver tilstand eller holdning som, etter PIs mening, ville hindre fullt samarbeid og engasjement for studien
