Detaljert beskrivelse
Hovedformålet med denne lokale, multisenterstudien er å undersøke genetiske og genuttrykksendringer i tumor før og etter progresjon på ribociclib, under kjernefasen og deretter før og etter progresjon på alpelisib og dermed identifisere mønstre av mutasjoner, hvordan de utvikler seg, og deres assosiasjon med CDK4/6-hemming og utfall som vedvarende respons eller tidlig progresjon. Studien tar også sikte på å evaluere farmakogenomikk og dens assosiasjon med uønskede hendelser (frekvens og alvorlighetsgrad), medikament-interaksjoner og kliniske utfall.
Til slutt vil studien også generere ytterligere langsiktige sikkerhets- og effektdata i denne spesifikke italienske befolkningen.

KJERNEFASE Inkluderingskriterier:
- Pasienten har en avansert (lokoregionalt tilbakevendende eller metastatisk) brystkreft i førstelinjebehandling (behandlingsnaiv for avansert setting).
- Pasienten er i postmenopause, definert av ett av følgende:
- Tidligere bilateral ooforektomi
- Alder ≥60
- Alder <60 og amenoré i 12 eller flere måneder (i fravær av kjemoterapi, tamoxifen, toremifen eller ovariesuppresjon) og FSH og østradiol i postmenopausalt område per lokalt normalområde
- Pasienten har en histologisk og/eller cytologisk bekreftet diagnose av østrogenreseptorpositiv og/eller progesteronreseptorpositiv brystkreft av lokalt laboratorium.
- Pasienten har en HER2-negativ brystkreft definert som en negativ in situ hybridiseringstest eller en IHC-status på 0, 1+ eller 2+. Hvis IHC er 2+, kreves en negativ in situ hybridiseringstest (FISH, CISH eller SISH) ved lokal laboratorietesting.
- Pasienten er villig til å gjennomgå blod- og tumorprøvetaking for de biologiske vurderingene/målene som er planlagt i protokollen.
Ekskluderingskriterier for KJERNEFASE:
- Pasient som tidligere har fått behandling med en hvilken som helst CDK4/6-hemmer.
- Pasient som tidligere har fått systemisk hormonbehandling eller kjemoterapi for avansert brystkreft.
Merk:
Pasienter som fikk neo/adjuvant behandling for brystkreft er kvalifisert. Hvis den tidligere neo/adjuvante behandlingen inkluderte letrozol eller anastrozol, må det sykdomsfrie intervallet være større enn 12 måneder fra avsluttet behandling til studiestart.
• Pasienter som fikk ≤ 28 dager med letrozol eller anastrozol for avansert sykdom før inkludering i denne studien er kvalifisert.
– Pasienten bruker for tiden annen kreftbehandling. Andre protokolldefinerte inkluderings-/ekskluderingskriterier kan gjelde.
UTVIDELSESFASE Inklusjonskriterier:
- Pasienten har blitt seponert (enhver tillatt grunn) fra behandling med ribociclib + letrozol i kjernefasen og anses egnet for behandling med alpelisib + fulvestrant i andre linje. Ribociclib + letrozol må være siste behandlingsregime før alpelisib + fulvestrant.
- Pasienten har PIK3CA-mutasjon som bestemt i tumorvev og/eller plasma av et Novartis-utpekt laboratorium. Resultater av vevsprøver tatt under kjernefasen (screening eller EOT) er akseptable
UTVIDELSESFASE Ekskluderingskriterier:
- Pasienten har tidligere fått behandling med PI3K-hemmere.
- Pasienten bruker samtidig annen kreftbehandling. Ribociclib og letrozol brukt i kjernefasen må seponeres minst 7 dager før dag én av forlengelsesstudiebehandlingen.
Alle legemidler med overlappende toksisiteter må seponeres innen 7 dager og AE oppløses til NCI CTCAE v4.03 Grade ≤1 før studiebehandling. Unntak fra dette kriteriet: pasienter med hvilken som helst grad av alopecia får delta i studien.

PR was defined as at least a 30% decrease in the sum of the longest diameter of target lesions, taking as reference the baseline sum longest diameter.
