Detaillierte Beschreibung
Der Hauptzweck dieser lokalen, multizentrischen Studie besteht darin, genetische und Genexpressionsveränderungen im Tumor vor und nach der Progression unter Ribociclib, während der Kernphase und dann vor und nach der Progression unter Alpelisib zu untersuchen und so Muster von Mutationen zu identifizieren, wie sie sich entwickeln, und ihre Assoziation mit CDK4/6-Hemmung und Ergebnissen wie anhaltendem Ansprechen oder früher Progression. Die Studie zielt auch darauf ab, die Pharmakogenomik und ihren Zusammenhang mit unerwünschten Ereignissen (Häufigkeit und Schweregrad), Arzneimittelwechselwirkungen und klinischen Ergebnissen zu bewerten.
Schließlich wird die Studie auch zusätzliche langfristige Sicherheits- und Wirksamkeitsdaten in dieser spezifischen italienischen Population generieren.

Beschreibung
KERNPHASE Einschlusskriterien:
- Die Patientin hat einen fortgeschrittenen (lokoregional rezidivierenden oder metastasierten) Brustkrebs in Erstlinienbehandlung (behandlungsnaiv für das fortgeschrittene Setting).
- Die Patientin befindet sich in der Postmenopause, definiert durch eines der folgenden:
- Vorherige bilaterale Ovarektomie
- Alter ≥60
- Alter <60 und Amenorrhoe für 12 oder mehr Monate (ohne Chemotherapie, Tamoxifen, Toremifen oder Ovarialsuppression) und FSH und Östradiol im postmenopausalen Bereich gemäß lokalem Normalbereich
- Der Patient hat eine histologisch und/oder zytologisch bestätigte Diagnose von Östrogenrezeptor-positivem und/oder Progesteronrezeptor-positivem Brustkrebs durch ein lokales Labor.
- Die Patientin hat einen HER2-negativen Brustkrebs, definiert als negativer In-situ-Hybridisierungstest oder einen IHC-Status von 0, 1+ oder 2+. Wenn IHC 2+ ist, ist ein negativer In-situ-Hybridisierungstest (FISH, CISH oder SISH) durch lokale Labortests erforderlich.
- Der Patient ist bereit, sich einer Blut- und Tumorprobenentnahme für die im Protokoll vorgesehenen biologischen Bewertungen/Ziele zu unterziehen.
KERNPHASE Ausschlusskriterien:
- Patient, der zuvor mit einem CDK4/6-Inhibitor behandelt wurde.
- Patientin, die zuvor eine systemische Hormontherapie oder Chemotherapie gegen fortgeschrittenen Brustkrebs erhalten hat.
Notiz:
Patientinnen, die eine neo/adjuvante Therapie gegen Brustkrebs erhalten haben, sind teilnahmeberechtigt. Wenn die vorherige neo/adjuvante Therapie Letrozol oder Anastrozol umfasste, muss das krankheitsfreie Intervall länger als 12 Monate vom Abschluss der Behandlung bis zum Studieneintritt sein.
• Patienten, die vor der Aufnahme in diese Studie ≤ 28 Tage Letrozol oder Anastrozol wegen fortgeschrittener Erkrankung erhalten haben, sind teilnahmeberechtigt.
- Der Patient wendet derzeit eine andere Krebstherapie an. Andere protokolldefinierte Einschluss-/Ausschlusskriterien können gelten.
ERWEITERUNGSPHASE Aufnahmekriterien:
- Die Behandlung mit Ribociclib + Letrozol in der Kernphase wurde aus beliebigem Grund abgesetzt und wird als geeignet für die Behandlung mit Alpelisib + Fulvestrant in der Zweitlinientherapie erachtet. Ribociclib + Letrozol muss das letzte Behandlungsschema vor Alpelisib + Fulvestrant sein.
- Der Patient hat eine PIK3CA-Mutation, wie in Tumorgewebe und/oder Plasma durch ein von Novartis benanntes Labor festgestellt wurde. Ergebnisse von Gewebeproben, die während der Kernphase (Screening oder EOT) gewonnen wurden, sind akzeptabel
ERWEITERUNGSPHASE Ausschlusskriterien:
- Der Patient hat zuvor eine Behandlung mit PI3K-Inhibitoren erhalten.
- Der Patient wendet gleichzeitig eine andere Krebstherapie an. Ribociclib und Letrozol, die in der Kernphase verwendet werden, müssen mindestens 7 Tage vor dem ersten Tag der Verlängerungsstudienbehandlung abgesetzt werden.
Alle Arzneimittel mit sich überschneidenden Toxizitäten müssen innerhalb von 7 Tagen abgesetzt werden und die UE müssen vor der Studienbehandlung auf NCI CTCAE v4.03 Grad ≤ 1 abgeklungen sein. Ausnahme von diesem Kriterium: Patienten mit Alopezie jeden Grades dürfen an der Studie teilnehmen.

PR was defined as at least a 30% decrease in the sum of the longest diameter of target lesions, taking as reference the baseline sum longest diameter.
SD was defined as neither sufficient shrinkage to qualify for PR nor sufficient increase to qualify for progressive disease (PD), taking as reference the smallest sum longest diameter since the treatment started.
