HPV-vaccination: Evaluering af påmindelser om dosis 2 og 3

Undersøgelsesholdet planlægger at udføre et randomiseret klinisk forsøg med tre arme: (1) sædvanlig pleje, (2) den simple påmindelse og (3) den simple påmindelse plus scriptet. Randomisering vil blive udført på lægeniveau på hvert klinisk sted for at sikre, at alle tre behandlingsarme er repræsenteret i hver klinik. Det primære resultat, HPV-vaccineaccept, vil blive vurderet for hver forsøgsdeltager.
Denne undersøgelse vil blive implementeret gennem Child Health Improvement through Computer Automation system (CHICA). CHICA er et computerbaseret klinisk beslutningsstøttesystem, der har fungeret i Wishard/Eskenazi-systemet siden 2004. CHICA opererer nu i 5 klinikker i sundhedssystemet og har betjent langt over 37.000 børn. CHICA understøtter primær pædiatrisk pleje, understøtter screening, rådgivning, forudgående vejledning, vaccinationer og kronisk sygdomsdiagnose og behandling.
CHICA er unik i flere henseender. Disse omfatter en unik skræddersyet, scanbar papirgrænseflade, en prioriteringsordning, der sikrer, at lægerne får højeste prioritet påmindelser, og muligheden for at indsamle data direkte fra familier i venteværelset. CHICA fungerer som frontend til Regenstrief Medical Record System (RMRS), den elektroniske journal for Eskenazi-sundhedssystemet. RMRS og CHICA kommunikerer ved hjælp af industristandard HL7 kommunikationsprotokoller.
Når en patient tjekker ind på en CHICA-klinik, modtager CHICA en HL7 ADT-meddelelse (registrering). Som svar anmoder CHICA om en download af patientjournalen fra RMRS. CHICA anvender et sæt Arden Syntax-regler på patientens data for at vælge 20 ja/nej-spørgsmål, der udskrives. Familien besvarer disse spørgsmål ved at afkrydse felter. Sygeplejersken eller lægeassistenten indtaster højde, vægt osv. på samme skema og skemaet scannes. Dataene kommer ind i CHICAs database gennem optisk mærkegenkendelse og optisk tegngenkendelse. For nylig er vi begyndt at konvertere fra dette scanbare papirformat til elektroniske tablets.

- Alle børnelæger og sygeplejersker, der yder sundhedspleje på de 5 CHICA-klinikker, vil blive inkluderet i denne undersøgelse
Fagkriterier
- Patienterne vil være 11-17 år
- Hanner og hunner
- Et besøg på mindst én af de fem CHICA-klinikker
- Berettiget til 2. eller 3. dosis af HPV-vaccine
Ekskluderingskriterier:
- Patienter i den berettigede aldersgruppe, som ikke er startet eller har afsluttet HPV-vaccineserien, vil blive udelukket
