Yksityiskohtainen kuvaus
Ehdotetun tutkimuksen yleistavoitteena on suorittaa prospektiivinen, satunnaistettu kontrolloitu tutkimus, jonka tarkoituksena on selvittää, parantaako BFR-hoidon lisääminen tavanomaiseen postoperatiiviseen kuntoutusohjelmaan merkittävästi kliinisiä tuloksia standardoidun ACLR:n jälkeen sekä kirurgisen tekniikan että siirteen valinnan suhteen. Tutkijan keskeinen hypoteesi on, että BFR-hoidon lisääminen voi nopeuttaa kirurgista toipumista, nopeuttaa paluuta työhön ja maksimoida lääketieteellisen valmiuden ACLR:n jälkeen. Ehdotetussa tutkimuksessa validoituja potilaiden raportoituja tulosmittauksia käytetään arvioimaan toiminnallista kliinistä paranemista, kun taas nelipäisen lihaksen poikkileikkauspinta-alaa (CSA), joka on mitattu MRI:llä, käytetään kvantifioimaan hoidon vaikutus nelipäisen reisilihaksen vahvuuteen. Tämän menetelmän käyttöperusteet perustuvat siihen tosiasiaan, että nelipäisen reisilihaksen CSA:n ja lihasten tilavuuden mittaaminen nelipäisen lihasten yleisen toiminnan korvikkeena on kuvattu hyvin, ja useat tutkimukset ovat vakuuttavasti osoittaneet positiivisen korrelaation näiden parametrien ja nelipäisen lihasten voimakkuuden välillä4, 17. Näistä raporteista huolimatta MRI:n hyväksyminen nelipäisen lihasvoiman korvikkeena on kuitenkin edelleen rajallinen. Siksi sekä leikkauksen että ei-leikkauksen polven ojentajalihasten (nelipäisten) isokineettinen ja isometrinen nelipäisen lihasvoima mitataan leikkauksen jälkeen noin 6 kuukauden ja 9 kuukauden kuluttua leikkauksesta. Kahdenvälisten reisien magneettikuvaus tehdään välittömästi ennen ACLR:ää perustason CSA:n määrittämiseksi leikkauksessa ja ei-leikkauksessa. Tämän jälkeen suoritetaan uusi magneettikuvaus välittömästi standardoidun 16 viikon leikkauksen jälkeisen kuntoutusohjelman päätyttyä potilaille, jotka on satunnaistettu joko normaaliin kuntoutusohjelmaan (REHAB) tai kuntoutusohjelmaan, jossa on BFR (REHAB + BFR). Lopuksi MRI toistetaan 52 viikon kuluttua leikkauksesta CSA:n piilevien muutosten tutkimiseksi sen jälkeen, kun potilaat on palautettu rajoittamattomaan fyysiseen toimintaan.
TAVOITE 1: Selvittää, lisääkö BFR-hoidon lisääminen tavalliseen leikkauksen jälkeiseen kuntoutusprotokollaan nelipäisen lihaksen poikkileikkausalaa ja tilavuutta ACLR:n jälkeen mitattuna magneettikuvauksella (MRI).
Hypoteesi: BFR-hoidon lisääminen tavanomaiseen leikkauksen jälkeiseen kuntoutusohjelmaan ACLR:n jälkeen lisää nelipäisen lihaksen CSA:ta ja volyymia verrattuna tavanomaiseen leikkauksen jälkeiseen kuntoutusohjelmaan, joka mitataan 16 viikon kuluttua leikkauksesta.
TAVOITE 2: Määrittää, jatkuuko havaittu nelipäisen lihaksen CSA:n ja tilavuuden kasvu BFR-hoidon lopettamisen jälkeen 16 viikon kuluttua leikkauksesta suorittamalla toistuva MRI 52 viikon kuluttua leikkauksesta
Hypoteesi: Havaittu lisääntyminen nelipäisen lihaksen CSA:ssa ja tilavuudessa säilyy BFR-hoidon lopettamisen jälkeenkin 52 viikkoa leikkauksen jälkeen.
TAVOITE 3: Selvittää, ennustavatko MRI:llä saadut CSA-mittaukset luotettavasti polven ojentajavoimaa.
Hypoteesi: MRI:llä saadut CSA-mittaukset ennustavat luotettavasti polven venyttäjän voimaa mitattuna isokineettisellä ja isometrisellä testillä.
TAVOITE 4: Selvittää, parantaako BFR-hoidon lisääminen tavanomaiseen leikkauksen jälkeiseen kuntoutusohjelmaan ACLR:n jälkeen merkittävästi parempia toiminnallisia tuloksia verrattuna tavanomaiseen leikkauksen jälkeiseen kuntoutusohjelmaan 24, 36 ja 52 viikon leikkauksen jälkeen käyttävällä potilaalla raportoidut tulokset (PRO).
Hypoteesi: BFR-hoidon lisääminen tavanomaiseen leikkauksen jälkeiseen kuntoutushoitoon ACLR:n jälkeen parantaa merkittävästi PRO:ta verrattuna tavanomaiseen leikkauksen jälkeiseen kuntoutusohjelmaan 24, 36 ja 52 viikon kuluttua leikkauksesta.

Kuvaus
Sisällyttämiskriteerit:
- Aktiivinen jäsen tai puolustusministeriön sotilasterveysjärjestelmän edunsaaja
- 18-40 vuoden iässä
- Indikoitu ACL-rekonstruktioon nelipäisen jänteen autograftilla.
Poissulkemiskriteerit:
- Potilaat, joille tehdään samanaikainen kirurginen toimenpide, joka muutoin vaatisi liikkumattomuutta ja/tai rajoitettua painon kantamista (esim. nivelkiven korjaus, nivelkierteen allograftin siirto, osteokondraalinen allograftisiirto, korkea sääriluun osteotomia) jätetään pois.
- Käynnissä yhdistetty multiligamenttinen polvivamman rekonstruktio
- Ei pysty jatkuvasti osallistumaan määrättyyn leikkauksen jälkeiseen kuntoutusohjelmaan
- Ei potilaita, joilla on ollut äskettäin alaraajojen syvä laskimotromboosi, 12 kuukauden sisällä tai jotka ovat saaneet aktiivista antikoagulanttihoitoa, joilla ei ole ollut ipsilateraalista alaraajojen imusolmukkeiden dissektiota tai endoteelin toimintahäiriötä.
- Potilaat, jotka eivät pysty saamaan magneettikuvausta joko intoleranssin ja/tai implantoitujen lääketieteellisten laitteiden vuoksi, jotka estävät magneettikuvauksen turvallisen suorittamisen.

The VAS is a unidimensional measure of pain intensity that has been frequently reported among adults.
The simplest VAS is represented as a horizontal 10 cm line.
The ends are defined as the extreme limits of parameter being measured (e.g.
pain) oriented from the left (worst) to the right (best.)
Patients are asked mark their perceived level of pain on the horizontal line.
The VAS score is determined by measuring in mm from the left hand end of the horizontal line to where the patient marks.
A higher score represents greater pain intensity.

This questionnaire is broken down into three categories: symptoms, sports activity, and knee function.
The symptoms subscale focuses on an assessment of pain, stiffness, swelling, and the sensation of "giving-way" of the knee.
The sports activity subscale focuses on completion of tasks such as going up and down stairs, rising from a seated position, squatting, and jumping.
The knee function subscale asks the respondent to characterize their knee at present versus prior to injury.
The IKDC is scored out of 100 points, with a score of 100 representing optimal knee function.

Patient Reported Outcome - Knee Osteoarthritis Outcome Score (KOOS)

The KOOS is a comprehensive scoring system that evaluates both short and long-term consequence of knee injury.
It is comprised of 42 items, which are scored separately in 5 subscales (pain (9 items), other symptoms (7 items), function in daily living (17 items), function in sport and recreation (5 items), and knee-related quality of life (5 items).)
Among the strengths of the KOOS as a PRO is its inclusion of two different subscales for physical function relating to daily life, sport, and recreation.
A score of 100 represents no knee symptoms, which is then converted to a percentage score to characterize a patient's overall satisfaction with their knee function.

This instrument has shown reliability in measuring the ability to adapt to change, the ability to deal with what comes along, the ability to cope with stress, the ability to stay focused and think clearly, the ability to not get discouraged in the face of failure, the ability to handle unpleasant feelings such as anger, pain or sadness.

Anterior Cruciate Ligament Return to Sport after Injury (ACL-RSI) (Short version 6Q)

ACL-RSI instrument measure one's emotion, confidence, and risk appraisal when returning to sports after an ACL injury and/or reconstructive surgery.
The survey consists of 12 items that are graded on a visual analogue scale from 0 points (extremely negative psychological responses) to 100 points (no negative psychological responses).
The results of the survey have been found to be strongly and significantly associated with return to sport.
