Biopuce Drépanocytose (SCD) : vers un moyen simple et fiable de surveiller la drépanocytose
L'anémie «en forme de faucille» a été décrite pour la première fois aux États-Unis en 1910, et la molécule d'hémoglobine falciforme héréditaire mutée a été identifiée en 1949. La physiopathologie de la SCD est une conséquence de la polymérisation anormale de l'hémoglobine falciforme (HbS) et de ses effets sur les propriétés, la forme et la densité de la membrane des globules rouges, ainsi que des changements critiques ultérieurs dans la fonction des cellules inflammatoires et des cellules endothéliales. Notre objectif est de comprendre l'impact des anomalies CMA dans la SCD, en interrogeant un certain nombre d'interactions reconnues dans une gamme de phénotypes cliniques.
À ce jour, les études corrélatives sur la SCD, par nous et d'autres, ont oscillé entre les rapports cliniques, basés sur des tests, des interventions et l'examen des dossiers d'individus ou de groupes d'individus et, à l'autre extrême, l'identification de polymorphismes génétiques fonctionnels basés sur des études de population. . Les chercheurs souhaitent compléter ces études par un examen systématique des propriétés de la membrane cellulaire et de l'état d'activation. Parmi les troubles hématologiques, le SCD peut être exceptionnellement sensible à un tel examen.

- Documentation Drépanocytose, y compris HbSS ou diagnostic HbSC- ou HbSβ-thalassémie composé hétérozygote, mis en évidence par une ou plusieurs caractéristiques cliniques.
- Consentement éclairé écrit (et consentement le cas échéant) obtenu du sujet ou du représentant légal du sujet et capacité du sujet à se conformer aux exigences de l'étude.
Critère d'exclusion
- Présence d'une condition ou d'une anomalie qui, de l'avis de l'investigateur, compromettrait la sécurité du patient ou la qualité des données.

Développer une biopuce SCD avec laquelle examiner les principales propriétés et interactions cellulaires, y compris les propriétés cellulaires, adhésives et inflammatoires des globules rouges et des globules blancs, ainsi que les caractéristiques des cellules précurseurs endothéliales et hématopoïétiques en circulation.

Adhésion des globules rouges et blancs aux biomolécules (par ex.
laminine, fibronectine et sélectines) seront mesurés sur un dispositif microfluidique, le SCD Biochip.
L'adhérence sera mesurée dans des conditions normales d'oxygène et de faible teneur en oxygène.
Les adhérences seront évaluées par rapport aux résultats cliniques, tels que les paramètres hématologiques et les preuves de vaso-occlusion, de douleur, d'inflammation et de vasculopathie chez les patients atteints de drépanocytose.
et des cellules précurseurs hématopoïétiques basées sur les propriétés et l'adhérence de la membrane.
