Surveillance du CHC : comparaison de la surveillance abrégée par IRM et échographie sans contraste chez les patients cirrhotiques avec une visualisation échographique sous-optimale
Toutes les directives internationales recommandent une surveillance échographique tous les 6 mois pour les patients à risque de cancer du foie (carcinome hépatocellulaire ou CHC), tels que les patients atteints de cirrhose. Le but de la surveillance est de détecter le CHC à un stade précoce alors qu'il est encore potentiellement curable. Actuellement, seuls 4 CHC sur 10 sont détectés à un stade précoce.
La surveillance échographique du CHC a une sensibilité étendue, dépendant de nombreux facteurs tels que l'expérience de l'opérateur, l'habitus corporel du patient et l'hétérogénéité parenchymateuse du foie due à une maladie hépatique chronique et à une cirrhose. Dans un groupe restreint de patients, l'échographie de surveillance peut être sous-optimale ou presque non diagnostique.
Actuellement, aucune ligne directrice ne propose un outil de surveillance alternatif pour les patients qui ont des échographies de surveillance sous-optimales.

La description
Critère d'intégration
- Les patients atteints de cirrhose (toutes les causes de cirrhose, à l'exception des causes obscures telles que la fibrose vasculaire ou congénitale) ET une vision réduite de leur foie à l'échographie (vB et vC).
Les critères de cirrhose peuvent être obtenus avec l'une des méthodes suivantes :
- Histologiquement par biopsie hépatique
- Signes antérieurs d'une maladie hépatique décompensée telle qu'ascite, encéphalopathie, varices ou péritonite bactérienne
Suspicion clinique de cirrhose PLUS l'un des éléments suivants :
- Preuve radiologique de changements morphologiques du foie et preuve d'hypertension portale aux examens US, CT ou IRM, y compris l'identification d'une nodularité de surface hépatique, d'une splénomégalie, de collatérales portales, de varices et d'ascites
- Fibroscan (élastographie transitoire) raideur médiane du foie > 12,5 kPa, le Fibroscan doit être réalisé par un technicien expérimenté et interprété par l'hépatologue
- Numération plaquettaire <100 (x10^9/L) sans autre cause
- Absence d'antécédents ou de suspicion actuelle de CHC - L'absence de CHC est définie par une échographie hépatique, une tomodensitométrie multiphase ou une IRM avec contraste dans les 6 mois précédant la surveillance
- Le patient est capable de se conformer aux visites prévues, aux plans d'évaluation et aux autres procédures d'étude de l'avis de l'investigateur
- Le patient est disposé à fournir un consentement éclairé écrit
Critère d'exclusion
- Contre-indications à l'IRM (défibrillateur, stimulateur cardiaque, corps étranger métallique, claustrophobie sévère, etc.)
- Contre-indications au gadolinium
- Âge supérieur à 85 ans ou inférieur à 20 ans
- Grossesse ou allaitement
- Toute autre condition qui, de l'avis de l'investigateur, rendrait le patient inapte à l'inscription à l'essai ou pourrait interférer avec l'achèvement de l'étude
