Description détaillée
Cette étude suivra une conception d'essai adaptative bayésienne à 4 bras. Au fur et à mesure que les patients seront recrutés, ils seront randomisés dans l'un des quatre bras.
L'étude sera menée au sein d'une structure commune pour permettre un recrutement et une analyse efficaces. La structure globale est une plate-forme adaptative à 4 bras d'interventions de santé mobile et de soutien à l'adhésion psychosociale informée par une approche de prestation de services différenciée (DSD).
L'objectif 1 est une étude adaptative des interventions de mHealth et de soutien à l'observance psychosociale utilisant une conception adaptative bayésienne pour permettre la comparaison des éléments de l'intervention séparément et en combinaison. Les participants à l'objectif 1 seront randomisés dans l'un des 4 bras et suivis mensuellement pendant les 6 mois d'intervention, puis jusqu'à la fin du traitement par téléphone, avec une visite supplémentaire en personne pour établir le résultat principal. Le résultat principal est un résultat clinique/biologique combiné à 12 mois décrit ci-dessous. L'hypothèse 1a utilise tous les participants tandis que 1b utilise uniquement ceux des bras d'intervention mHealth (3 + 4) car une adhésion granulaire mesurée par EDM est requise.

Critère d'intégration:
- Culture ou test moléculaire positif pour MTB
- Test moléculaire positif pour le VIH ou antécédents documentés de séropositivité
- Tests de sensibilité aux médicaments par analyse moléculaire (c.-à-d. GeneXpert MTB/RIF) ou des tests conventionnels compatibles avec au moins une tuberculose résistante à la rifampicine,
- Initier un traitement avec un régime antituberculeux contenant de la bédaquiline dans les 4 semaines suivant l'inscription et le premier traitement avec BDQ
- Sous traitement avec un régime de thérapie antirétrovirale (ART), y compris une combinaison contenant du dolutégravir Régime de thérapie antirétrovirale (c.-à-d. Tenofovir-Lamivudine-Dolutergravir), ou commençant dans les 4 semaines suivant l'inscription,
- Capacité de consentement éclairé en isiZulu ou en anglais
