Szczegółowy opis
To badanie będzie przebiegać zgodnie z 4-ramiennym Bayesowskim, adaptacyjnym projektem próbnym. Gdy pacjenci zostaną włączeni, zostaną losowo przydzieleni do jednej z czterech grup.
Badanie zostanie przeprowadzone w ramach wspólnej struktury, aby umożliwić skuteczną rekrutację i analizę. Ogólna struktura to 4-ramienna adaptacyjna platforma interwencji wspierających m-zdrowie i przestrzeganie zaleceń psychospołecznych, oparta na podejściu zróżnicowanego świadczenia usług (DSD).
Cel 1 to adaptacyjne badanie interwencji wspierających m-zdrowie i przestrzeganie zasad psychospołecznych z wykorzystaniem adaptacyjnego projektu bayesowskiego, aby umożliwić porównanie elementów interwencji osobno iw połączeniu. Uczestnicy Celu 1 zostaną losowo przydzieleni do jednej z 4 grup i obserwowani co miesiąc przez 6 miesięcy interwencji, a następnie telefonicznie do końca leczenia, z dodatkową wizytą osobistą w celu ustalenia głównego wyniku. Pierwszorzędowy wynik to połączony wynik kliniczny/biologiczny po 12 miesiącach opisany poniżej. Hipoteza 1a obejmuje wszystkich uczestników, podczas gdy hipoteza 1b obejmuje tylko tych z grup interwencji mZdrowie (3+4), ponieważ wymagane jest szczegółowe przestrzeganie zaleceń mierzonych za pomocą EDM.

Kryteria przyjęcia:
- Hodowla lub test molekularny pozytywny dla MTB
- Pozytywny wynik testu molekularnego na obecność wirusa HIV lub udokumentowana historia zakażenia wirusem HIV
- Badanie lekowrażliwości metodą molekularną (tj. GeneXpert MTB/RIF) lub konwencjonalne testy zgodne co najmniej z gruźlicą oporną na ryfampicynę,
- Rozpoczęcie leczenia schematem gruźlicy zawierającym Bedakilinę w ciągu 4 tygodni od włączenia i pierwsze leczenie BDQ
- Podczas leczenia według schematu terapii przeciwretrowirusowej (ART), w tym złożonego schematu terapii przeciwretrowirusowej zawierającego dolutegrawir (tj. tenofowir-lamiwudyna-dolutergrawir) lub rozpoczęcie w ciągu 4 tygodni od włączenia,
- Zdolność do świadomej zgody w języku isiZulu lub angielskim
