A Von Willebrand-kór (VWD) a leggyakoribb öröklött vérzési rendellenesség, amelyet a Von Willebrand-faktor (VWF) mennyiségi és/vagy minőségi hiánya jellemez, amely jelentős szerepet játszik a hemosztázis korai szakaszában. A 3-as típusú von Willebrand-kór (VWD3) a VWF gyakorlatilag teljes hiányának köszönhető, és emiatt „súlyos VWD-ként” is leírták. A visszatérő gyomor-bélrendszeri vérzés (GIB) az egyik legnagyobb kihívást jelentő szövődmény a VWD-ben szenvedő betegek kezelése során. A leggyakoribb ok az angiodysplasia (ANGDYS), de gyakran nem azonosítják az okot a diagnózis felállításának nehézsége miatt. Az elmúlt években több laboratóriumban végzett kutatások a VWF számos szerepét azonosították az érrendszeri működés szabályozásában. Globálisan ezek az eredmények adják az első lehetséges magyarázatot az ANGDYS jelenlétére VWD-ben szenvedő betegeknél. A gasztrointesztinális (GI) traktus ezen érrendszeri rendellenességeit törékeny, szivárgó nyálkahártya-erek jellemzik. A hemosztatikus diszfunkcióval kombinálva ezek súlyos, kezelhetetlen vérzésekhez vezethetnek, beleértve a GIB-t is. A VWD3 recesszív tulajdonságként öröklődik, és a heterozigóta rokonoknál enyhe vérzéses tünetek jelentkeznek, vagy nincsenek is. Még ha a VWD3 prevalenciája nagyon alacsony is, a legmagasabb arány Iránban, a legalacsonyabb Dél-Európában található. A VWD3 tényleges prevalenciája azonban a legtöbb országban még mindig ismeretlen a retrospektív vagy prospektív tanulmányok hiánya miatt. Bár ritka, a VWD3 súlyos klinikai megjelenése, a plazmából származó és/vagy rekombináns VWF koncentrátumokkal történő helyettesítő terápia szükségessége, valamint a VWF koncentrátumok infúziója utáni anti-VWF gátlók előfordulásának kockázata miatt komoly érdeklődésre tart számot. tényezőket nem határozták meg szisztematikusan.
A tanulmány fő céljai: nemzetközi hálózat létrehozása az európai és iráni központok között (1:1 arány), legalább 250 VWD3-beteg várható felvétele egy közös online adatbázis segítségével, részletes információk gyűjtése a korábbi vérzésekről és expozícióról. plazmaeredetű és/vagy rekombináns VWF koncentrátumokra, közös kérdőívvel számított VWD3 vérzés súlyossági pontszámának felhasználása, plazma- és DNS minták gyűjtése az összes azonosított VWD3 betegtől centralizált elemzésre, a lokális VWD3 diagnózis megerősítése központosított tesztek segítségével, VWF génhibák, VWF fenotípus és az anti-VWF inhibitorok kockázatának értékelése közös módszerekkel, a fenotípusos eredmények (beleértve az angiogenezis markereit) és a GIB előfordulása közötti lehetséges összefüggések értékelése, a GIB súlyosságának objektív értékelése VWD3-ban betegeknél, a vérzés gyakoriságának és helyének felmérése VWD3-betegeknél, akiket 2 prospektív megfigyelési időszakon keresztül követtek (2 évenként: 2017-2018 és 2020-2022), a felhasznált plazmaeredetű és/vagy rekombináns VWF koncentrátumok hatékonyságának értékelése a VWD3 kezelésére (igény szerint kontra profilaxis) a legobjektívebb hatékonysági kritériumok alkalmazásával 2 prospektív megfigyelési időszakban (mindegyik 2 év: 2017-2018 és 2020-2022), a plazmaeredetű és/vagy rekombináns szerek hatékonyságának és biztonságosságának értékelése A VWF a GIB kezelésében 2 prospektív megfigyelési periódusra koncentrál (mindegyik 2 év: 2017-2018 és 2020-2022), összehasonlítva az antiangiogenetikus szerek standard klinikai környezetben történő használatával.
Ebből a célból legalább 250 VWD3-val diagnosztizált betegből álló csoportot vesznek fel homogén és szabványosított kritériumok alapján.
A projekt céljainak elérése érdekében tervezett munka három részre oszlik:
- az első rész a felvételre és a retrospektív klinikai és laboratóriumi adatok összegyűjtésére vonatkozó szabványosított kritériumokkal foglalkozik, amelyeket a központosított laboratóriumok megerősítenek;
- a második rész a retrospektív fázisban gyűjtött klinikai és laboratóriumi paraméterek további jellemzését foglalja magában, ideértve az anti-VWF-inhibitorok előfordulását, a VWD3 további azonosítására szolgáló fejlett laboratóriumi teszteket, a VWF-gén mutációinak elemzését;
- a tanulmány harmadik része két részre oszlik: egy első prospektív megfigyelésre és egy második prospektív megfigyelésre. A harmadik rész első ízben foglalkozik a VWD3-betegek nagy csoportjának prospektív klinikai megfigyelésével, amelyeket egy nemzetközi szakértői testület korábban jól jellemez.
