Choroba von Willebranda (VWD) jest najczęstszą wrodzoną skazą krwotoczną, charakteryzującą się ilościowym i/lub jakościowym niedoborem czynnika von Willebranda (VWF), który odgrywa główną rolę we wczesnych fazach hemostazy. Choroba von Willebranda typu 3 (VWD3) jest spowodowana praktycznie całkowitym niedoborem VWF iz tego powodu została również opisana jako „ciężkie VWD”. Nawracające krwawienia z przewodu pokarmowego (GIB) są jednym z najtrudniejszych powikłań napotykanych w leczeniu pacjentów z VWD. Najczęstszą przyczyną jest angiodysplazja (ANGDYS), ale często nie udaje się ustalić przyczyny ze względu na trudności w postawieniu diagnozy. W ostatnich latach badania kilku laboratoriów zidentyfikowały wiele ról VWF w kontroli czynności naczyń. Ogólnie rzecz biorąc, odkrycia te dostarczają pierwszego możliwego wyjaśnienia obecności ANGDYS u pacjentów z VWD. Te malformacje naczyniowe w przewodzie pokarmowym (GI) charakteryzują się kruchymi, nieszczelnymi naczyniami błony śluzowej. W połączeniu z dysfunkcją hemostazy może to prowadzić do ciężkiego, trudnego do opanowania krwawienia, w tym GIB. VWD3 jest dziedziczona jako cecha recesywna, a heterozygotyczni krewni mają łagodne objawy krwawienia lub nie mają ich wcale. Nawet jeśli częstość występowania VWD3 jest bardzo niska, najwyższy wskaźnik występuje w Iranie, a najniższy w południowej Europie. Jednak rzeczywista częstość występowania VWD3 jest nadal nieznana w większości krajów ze względu na brak badań retrospektywnych lub prospektywnych. VWD3, choć rzadki, cieszy się dużym zainteresowaniem ze względu na ciężki obraz kliniczny, konieczność stosowania terapii zastępczej koncentratami VWF pochodzącymi z osocza i/lub rekombinowanymi oraz ryzyko wystąpienia inhibitorów anty-VWF po infuzji koncentratów VWF, dla których ryzyko czynniki nie zostały ustalone w sposób systematyczny.
Głównymi celami badania są: stworzenie międzynarodowej sieci pomiędzy ośrodkami europejskimi i irańskimi (stosunek 1:1), prospektywna rejestracja co najmniej 250 pacjentów z VWD3 za pomocą wspólnej bazy danych online, zebranie szczegółowych informacji o przebytych krwawieniach i ekspozycji na koncentraty VWF pochodzące z osocza i/lub rekombinowanego, wykorzystanie oceny ciężkości krwawienia VWD3 obliczonej za pomocą wspólnego kwestionariusza, pobranie próbek osocza i DNA od wszystkich zidentyfikowanych pacjentów z VWD3 włączonych do analiz centralnych, potwierdzenie lokalnego rozpoznania VWD3 za pomocą testów scentralizowanych, ocena defektów genów VWF, fenotypu VWF i ryzyka stosowania inhibitorów anty-VWF wspólnymi metodami, ocena potencjalnych korelacji między wynikami fenotypowymi (w tym markerami angiogenezy) a występowaniem GIB, obiektywna ocena ciężkości GIB w VWD3 pacjentów, ocena częstości i miejsc krwawień u chorych na VWD3 obserwowanych przez 2 prospektywne okresy obserwacji (każdy po 2 lata: 2017-2018 i 2020-2022), ocena skuteczności zastosowanych koncentratów osoczowych i/lub rekombinowanych VWF w leczeniu VWD3 (na żądanie vs profilaktyka) przy zastosowaniu najbardziej obiektywnych kryteriów skuteczności podczas 2 prospektywnych okresów obserwacji (każdy po 2 lata: 2017-2018 i 2020-2022), ocena skuteczności i bezpieczeństwa osoczowych i/lub rekombinowanych VWF koncentruje się na leczeniu GIB w 2 prospektywnych okresach obserwacji (każdy po 2 lata: 2017-2018 i 2020-2022), w porównaniu ze stosowaniem leków antyangiogennych w standardowych warunkach klinicznych.
W tym celu kohorta co najmniej 250 pacjentów z rozpoznaniem VWD3 zostanie włączona przy użyciu jednorodnych i wystandaryzowanych kryteriów.
Prace zaplanowane dla osiągnięcia celów projektu zostaną podzielone na trzy części:
- pierwsza część dotyczy ustandaryzowanych kryteriów rekrutacji i gromadzenia retrospektywnych danych klinicznych i laboratoryjnych, które mają być potwierdzone przez scentralizowane laboratoria;
- druga część obejmuje dalszą charakterystykę parametrów klinicznych i laboratoryjnych zebranych w fazie retrospektywnej, w tym rozpowszechnienie inhibitorów anty-VWF, zaawansowane testy laboratoryjne w celu dalszej identyfikacji VWD3, analizy mutacji genu VWF;
- trzecia część badania jest podzielona na dwie części: pierwszą obserwację prospektywną i drugą obserwację prospektywną. Trzecia część po raz pierwszy dotyczy prospektywnej obserwacji klinicznej dużej kohorty pacjentów z VWD3, z których wszyscy zostali wcześniej dobrze scharakteryzowani przez międzynarodowy zespół ekspertów.

- Mężczyźni i kobiety w każdym wieku, w tym niemowlęta, dzieci, młodzież i dorośli
- Uzyskano świadomą zgodę (rodzice powinni podpisać w przypadku pacjentów <18 lat)
- Wcześniejsza diagnoza VWD3 (antygen VWF: niewykrywalny lub <5 U/dL)
- Szczegółowe informacje na temat dziedzicznego wzorca, historii krwawień, wcześniejszej ekspozycji na produkty krwiopochodne
- Dostępność próbek osocza i DNA
Kryteria wyłączenia:
• Pacjenci z VWD3, którzy mogą nie być dostępni do obserwacji
