A Doença de Von Willebrand (VWD) é o distúrbio hemorrágico hereditário mais comum, caracterizado por uma deficiência quantitativa e/ou qualitativa do Fator de Von Willebrand (VWF), que desempenha um papel importante nas fases iniciais da hemostasia. A doença de Von Willebrand tipo 3 (VWD3) é devida à deficiência virtualmente completa de VWF e, por esse motivo, também foi descrita como "vWD grave". Hemorragias gastrointestinais recorrentes (SGI) são uma das complicações mais desafiadoras encontradas no tratamento de pacientes com VWD. A causa mais comum é a angiodisplasia (ANGDYS), mas muitas vezes nenhuma causa é identificada devido à dificuldade no diagnóstico. Nos últimos anos, pesquisas de vários laboratórios identificaram múltiplos papéis para o VWF no controle da função vascular. Globalmente, esses achados fornecem a primeira explicação possível para a presença de ANGDYS em pacientes com DVW. Essas malformações vasculares no trato gastrointestinal (GI) são caracterizadas por vasos da mucosa frágeis e permeáveis. Combinados com a disfunção hemostática, podem levar a sangramento intratável grave, incluindo SGI. O VWD3 é herdado como um traço recessivo e os parentes heterozigotos apresentam sintomas leves ou inexistentes de sangramento. Mesmo que a prevalência de VWD3 seja muito baixa, a taxa mais alta é encontrada no Irã e a mais baixa no sul da Europa. No entanto, a prevalência real de VWD3 ainda é desconhecida na maioria dos países, devido à falta de estudos retrospectivos ou prospectivos. Embora raro, o VWD3 é de grande interesse devido à sua apresentação clínica grave, à necessidade de terapia de reposição com concentrados de VWF derivados do plasma e/ou recombinantes e ao risco de ocorrência de inibidores anti-VWF após a infusão de concentrados de VWF, para os quais o risco fatores não foram sistematicamente determinados.
Os principais objetivos do estudo são: criar uma rede internacional entre os centros europeus e iranianos (proporção 1:1), a inscrição prospectiva de pelo menos 250 pacientes VWD3 usando um banco de dados comum online, a coleta de informações detalhadas sobre sangramentos anteriores e exposição a concentrados de VWF derivados de plasma e/ou recombinantes, o uso do escore de gravidade do sangramento de VWD3 calculado com um questionário comum, a coleta de amostras de plasma e DNA de todos os pacientes VWD3 identificados inscritos para análises centralizadas, a confirmação do diagnóstico local de VWD3 usando testes centralizados, Avaliação de defeitos do gene VWF, fenótipo VWF e risco de inibidores anti-VWF através de métodos comuns, avaliação de potenciais correlações entre resultados fenotípicos (incluindo marcadores de angiogênese) e ocorrência de SGI, avaliação objetiva da gravidade de SGI em VWD3 pacientes, avaliação da frequência e locais de sangramento em pacientes com VWD3 acompanhados por 2 períodos de observação prospectiva (2 anos cada: 2017-2018 e 2020-2022), avaliação da eficácia dos concentrados de VWF derivados de plasma e/ou recombinantes usados para tratar VWD3 (sob demanda versus profilaxia) usando os critérios mais objetivos de eficácia durante 2 períodos de observação prospectiva (2 anos cada: 2017-2018 e 2020-2022), a avaliação da eficácia e segurança de derivados de plasma e/ou recombinantes O VWF concentra-se no tratamento de GIB durante 2 períodos de observação prospectiva (2 anos cada: 2017-2018 e 2020-2022), em comparação com o uso de agentes antiangiogênicos no cenário clínico padrão.
Para esses fins, uma coorte de pelo menos 250 pacientes com diagnóstico de VWD3 será incluída usando critérios homogêneos e padronizados.
O trabalho previsto para atingir os objetivos do projeto será dividido em três partes:
- a primeira trata de critérios padronizados para captação e coleta de dados clínicos e laboratoriais retrospectivos, a serem confirmados por laboratórios centralizados;
- a segunda parte envolve uma caracterização adicional dos parâmetros clínicos e laboratoriais, recolhidos na fase retrospectiva, incluindo prevalência de inibidores anti-VWF, testes laboratoriais avançados para identificação adicional de VWD3, análises de mutações do gene VWF;
- a terceira parte do estudo é dividida em duas partes: uma primeira observação prospectiva e uma segunda observação prospectiva. A terceira parte trata pela primeira vez da observação clínica prospectiva em uma grande coorte de pacientes com VWD3, todos previamente bem caracterizados por um painel internacional de especialistas.

- Diagnóstico prévio de VWD3 (Antígeno VWF: indetectável ou <5 U/dL)
- Informações detalhadas sobre padrão hereditário, histórico de sangramento, exposição prévia a hemoderivados
- Disponibilidade de amostras de plasma e DNA
Critério de exclusão:
• Pacientes VWD3 que podem não estar disponíveis para acompanhamento

Foi calculado o número de pacientes pertencentes a cada grupo de perfis multímeros (1 - Homozigotos / 2 - Apenas Protômeros / 3 - 2-4 Bandas).
A avaliação qualitativa dos multímeros do VWF faz parte do processo de diagnóstico do VWD3.
