La maladie de Von Willebrand (MVW) est le trouble hémorragique héréditaire le plus courant, caractérisé par un déficit quantitatif et/ou qualitatif du facteur Von Willebrand (VWF), qui joue un rôle majeur dans les phases précoces de l'hémostase. La maladie de von Willebrand de type 3 (VWD3) est due à une déficience quasi-totale en VWF et, pour cette raison, a également été décrite comme une "mvW sévère". Les saignements gastro-intestinaux récurrents (GIB) sont l'une des complications les plus difficiles rencontrées dans la prise en charge des patients atteints de la maladie de von Willebrand. La cause la plus fréquente est l'angiodysplasie (ANGDYS), mais souvent aucune cause n'est identifiée en raison de la difficulté à poser le diagnostic. Ces dernières années, la recherche de plusieurs laboratoires a identifié de multiples rôles pour le VWF dans le contrôle de la fonction vasculaire. Globalement, ces résultats fournissent la première explication possible de la présence d'ANGDYS chez les patients atteints de la maladie de von Willebrand. Ces malformations vasculaires du tractus gastro-intestinal (GI) sont caractérisées par des vaisseaux muqueux fragiles et qui fuient. Combinés au dysfonctionnement hémostatique, ceux-ci peuvent entraîner des saignements réfractaires graves, y compris GIB. VWD3 est hérité comme un trait récessif et les parents hétérozygotes ont des symptômes hémorragiques légers ou inexistants. Même si la prévalence de VWD3 est très faible, le taux le plus élevé se trouve en Iran et le plus bas en Europe du Sud. Cependant, la prévalence réelle de VWD3 est encore inconnue dans la plupart des pays, en raison du manque d'études rétrospectives ou prospectives. Bien que rare, le VWD3 présente un intérêt majeur en raison de sa présentation clinique sévère, de la nécessité d'un traitement substitutif par des concentrés de VWF dérivés du plasma et/ou recombinants et du risque de survenue d'inhibiteurs anti-VWF après la perfusion de concentrés de VWF, risque pour lequel facteurs n'ont pas été systématiquement déterminés.
Les principaux objectifs de l'étude sont : la création d'un réseau international entre les centres européens et iraniens (ratio 1:1), l'inscription prospective d'au moins 250 patients atteints de la maladie de von Willebrand 3 à l'aide d'une base de données commune en ligne, la collecte d'informations détaillées sur les saignements antérieurs et l'exposition aux concentrés de VWD dérivés du plasma et/ou recombinants, l'utilisation du score de gravité des saignements de VWD3 calculé avec un questionnaire commun, la collecte d'échantillons de plasma et d'ADN de tous les patients VWD3 identifiés enrôlés pour des analyses centralisées, la confirmation du diagnostic local de VWD3 à l'aide de tests centralisés, évaluation des défauts du gène VWF, du phénotype du VWF et du risque d'inhibiteurs anti-VWF par des méthodes courantes, l'évaluation des corrélations potentielles entre les résultats phénotypiques (y compris les marqueurs de l'angiogenèse) et la survenue de GIB, l'évaluation objective de la gravité du GIB dans VWD3 patients, l'évaluation de la fréquence et des sites de saignement chez les patients VWD3 suivis pendant 2 périodes d'observation prospectives (2 ans chacune : 2017-2018 et 2020-2022), l'évaluation de l'efficacité des concentrés de VWF dérivés du plasma et/ou recombinants utilisés pour traiter la MvW3 (à la demande versus prophylaxie) en utilisant les critères d'efficacité les plus objectifs pendant 2 périodes d'observation prospectives (2 ans chacune : 2017-2018 et 2020-2022), l'évaluation de l'efficacité et de la sécurité des dérivés plasmatiques et/ou recombinants VWF se concentre sur le traitement du GIB pendant 2 périodes d'observation prospectives (2 ans chacune : 2017-2018 et 2020-2022), par rapport à l'utilisation d'agents anti-angiogénétiques dans le cadre clinique standard.
À ces fins, une cohorte d'au moins 250 patients avec un diagnostic de VWD3 sera recrutée en utilisant des critères homogènes et standardisés.
Les travaux prévus pour atteindre les objectifs du projet seront divisés en trois parties :
- la première partie traite des critères normalisés d'inscription et de collecte des données cliniques et de laboratoire rétrospectives, à confirmer par les laboratoires centralisés ;
- la deuxième partie implique une caractérisation plus poussée des paramètres cliniques et de laboratoire, recueillis dans la phase rétrospective, y compris la prévalence des inhibiteurs anti-VWF, des tests de laboratoire avancés pour mieux identifier VWD3, des analyses de mutations du gène VWF ;
- la troisième partie de l'étude est divisée en deux parties : une première observation prospective et une seconde observation prospective. La troisième partie traite pour la première fois de l'observation clinique prospective dans une large cohorte de patients VWD3 tous préalablement bien caractérisés par un panel international d'experts.

L'analyse multimère du facteur Von Willebrand (VWF) a été réalisée par électrophorèse d'échantillons sanguins prélevés par des sites expérimentaux.
Le nombre de patients appartenant à chaque groupe de profils multimères (1 - Homozygotes / 2 - Uniquement Protomères / 3 - 2-4 Bandes) a été calculé.
L'évaluation qualitative des multimères VWF fait partie du processus de diagnostic de VWD3.
