상세 설명
근거: OSA는 예후가 좋지 않은 CAD의 일반적인 상태입니다. 이러한 피험자 중 다수는 주간 졸음을 보고하지 않으므로 CPAP로 OSA 치료를 고려하지 않습니다. OSA가 있는 CAD 환자의 장기 예후에 대한 CPAP의 영향에 대한 증거가 부족합니다.
목적: RICCADSA 시험은 CPAP 치료가 OSA(무호흡-저호흡-지수) CAD 환자의 평균 3년 추적 관찰 기간 동안 새로운 혈관재생술, 심근경색, 뇌졸중 및 심혈관계 사망률의 결합률을 감소시키는지 여부를 다루도록 설계되었습니다. AHI]>=15/h) 주간 졸림 없음(Epworth Sleepiness Scale[ESS]<10). 2차 결과에는 심혈관 바이오마커, 심장 기능, 기준선에서의 최대 운동 능력 및 삶의 질, 3개월 및 1년 추적 조사, 수면다원검사 소견 및 CPAP 요법 준수가 포함됩니다.
환자 및 방법: 511명의 CAD 환자 샘플(CPAP에 무작위 배정된 비졸림 OSA 환자 122명, 비CPAP에 122명, CPAP를 사용하는 졸린 OSA 환자 155명[ESS>=10], OSA가 없는 CAD 환자 112명[AHI <5 시간당])이 포함되었습니다. 임상시험은 치료 의도 기반의 주요 결과에 대해 25%에서 12%로 가정된 복합 종점 비율에서 위험 감소를 감지할 수 있는 80%의 검정력을 가집니다.

포함 기준:
- PCI 또는 CABG 치료를 새로 받은 혈관 조영술로 확인된 CAD 환자
- 서면, 정보에 입각한 연구 동의서
- OSA(AHI>=15/시간) 또는 non-OSA(AHI<5/시간) 집에서 무인 수면 기록으로 진단
제외 기준:
- 이미 치료받은 OSAS 환자
- 주로 중추성 무호흡(Cheynes-Stokes 호흡)을 나타내는 환자
- 집에서 무인 수면 기록 시 경계선 OSA(AHI <15 및 시간당 >=5) 환자
