안드로겐 결핍이 있는 전립선암 생존자의 삶의 질 향상

이 2상 시험의 목적은 근치적 전립선 절제술을 받은 전립선암 남성의 안드로겐 결핍 증상 및 건강 관련 삶의 질을 개선하는 테스토스테론 대체 요법(TRT)의 효능과 안전성을 결정하는 것입니다.

전반적인 목표는 안드로겐 결핍 증상(성적 증상, 낮은 장기 국소 질환(pT2,N0,M0), Gleason 점수 < 7(3+4)에 대한 근치적 전립선 절제술을 받은 전립선암 남성의 전반적인 건강 관련 삶의 질, 근치적 전립선 절제술 후 > 2년 동안 PSA(민감한 PSA 분석을 사용하여 <0.1 ng/mL)하고 안드로겐 결핍이 있는 사람.

단계 pT2, N0, M0 병변(샘에 국한됨)이 있는 전립선암을 가진 남성; 결합된 Gleason 점수 7(3+4 요소) 이하; 10 ng/ml 미만의 수술 전 PSA; 근치적 전립선 절제술 후 최소 2년 동안 안정적이고 감지할 수 없는 PSA 수치(기능적 민감도가 0.1ng/mL인 분석법을 사용하여 PSA가 0.1ng/mL 미만).
- 연령: 40세 이상
- 성기능 장애, 피로/활력 저하 또는 신체 기능 장애와 관련된 증상의 유무.
- LC-MS/MS로 측정한 평균 2회의 공복, 이른 아침 혈청 테스토스테론 수치, 275mg/dL 미만 및/또는 평형 투석에 의한 유리 테스토스테론 <60pg/mL. 평균 테스토스테론 수치가 > 300 ng/dL인 중년 및 노년 남성.

- 방사선 치료를 받은 남성
- 안드로겐 차단 요법을 받는 남성은 제외됩니다.
- 헤모글로빈 <10g/dL 또는 >16.5g/dL
- 치료받지 않은 심각한 수면 무호흡증
- 참기름 알레르기
- 조절되지 않는 심부전
- 3개월 이내의 심근경색, 급성관상동맥증후군, 혈관재생술 또는 뇌졸중
- 혈청 크레아티닌 >2.5 mg/dL; ALT 3x 정상 상한;
- 헤모글로빈 A1c >7.5% 또는 인슐린 요법이 필요한 당뇨병
- 체질량 지수(BMI) >40kg/m2
- 치료되지 않은 우울증. 안정적인 항우울제를 복용했거나 3개월 이상 CBT를 받고 있는 우울증 환자가 대상입니다.
- 정신 분열증과 같은 축 I 정신 장애가 있는 남성은 제외됩니다.
- 지난 6개월 이내에 다음 약물을 사용한 피험자: 테스토스테론, DHEA, 에스트로겐, GnRH 유사체, 항안드로겐, 스피로노락톤, 케토코나졸, rhGH, 메게스트롤 아세테이트, 프레드니손 1일 20mg 또는 2주 이상 다른 글루코코르티코이드와 동등한 용량

Biochemical recurrence is defined as two or more consecutive serum PSA levels of 0.2 ng/mL or higher.

Change From Baseline in Overall Sexual Activity, Assessed Using Psychosexual Diary Questionnaire (PDQ) Question 4

The primary efficacy endpoint is the change from baseline in overall sexual activity, assessed using Psychosexual Diary Questionnaire (PDQ) Question 4. Subjects complete the PDQ questionnaire daily for 7 consecutive days before each visit.
The PDQ covers 3 domains:1) sexual desire, enjoyment, and performance; 2) sexual activity score; and 3) mood.
Sexual activity (PDQ-4) is assessed using a checklist of 12 sexual activities including sexual interactions, masturbation, intercourse, erection, orgasm, and ejaculation on each of the 7 days.
The value is recorded as 0 (none) or 1 (any) for each of the sexual activities.
Weekly value is the sum of "any" responses for the week.
The sexual activity score is the average of the weekly values.
The score ranges from 0 to 12, with 0 indicating no activity had taken place that week and with higher values indicating more sexual activity.

The IPSS is a validated 8-question survey (7 symptom questions, 1 quality-of-life question) used to assess the severity of lower urinary tract symptoms (LUTS) associated with BPH, ranging from 0-35, with a higher score indicating a more severe negative symptoms.

The secondary efficacy endpoint is sexual desire, assessed by the DeRogatis Inventory of Sexual Function Sexual Desire (DISF-SD) Questionnaire.
The DISF evaluates an individual's sexual health across five primary domains: sexual cognition/fantasy, arousal, behavior/experience, orgasm, and sexual drive/relationship.
The Sexual Desire (SD) subset assesses an individual's subjective libido and frequency of sexual drive.
The Sexual Desire score can range from 0 to 33, with a higher score indicating more sexual desire.

The secondary efficacy endpoint is erectile function will be assessed by the International Index of Erectile Function (IIEF) Questionnaire.
The IIEF is a 15-item questionnaire that covers 4 domains of sexual function: erectile function, sexual desire, orgasmic function, and intercourse satisfaction.
Each question has a potential score of 0 to 5 for a maximal score of 75 for the entire scale.
Our focus in this study is on erectile function domain score, which can range from 0 to 30, lower scores indicating poor erectile function, and higher score indicating better erectile function.

The secondary efficacy endpoint is energy level, assessed using the Hypogonadism Energy Diary (HED).
The total HED scale score is the sum of each of the six item weekly scores, with higher scores corresponding to greater energy level.
The HED total score is linearly transformed to a scale of 0-100.
Lower scores indicate lower energy (or more tiredness) and higher scores indicate more energy (less tiredness).

Mood and well-being will be assessed by the Positive and Negative Affect Schedule (PANAS).
The PANAS is a 20-item self-report measure to assess positive affect (PA) and negative affect (NA), with scores ranging from 10 to 50 for both the PA score and the NA score.
For the PA score (10-50), higher scores represent higher levels of positive emotions, engagement, and enthusiasm.
For the NA score (10-50), higher scores indicate higher levels of negative engagement, distress, and anxiety.

The secondary efficacy endpoint is change from baseline in prostate cancer-related quality of life measured using the hormonal and sexual domains of the Expanded Prostate Cancer Index Composite (EPIC).
Expanded Prostate Cancer Index Composite Instrument (EPIC-26) for Measuring Health-Related Quality of Life Among Prostate Cancer Survivors EPIC QOL is a 26-item questionnaire that assesses quality of life in 5 domains: urinary incontinence, urinary irritation/obstruction, bowel, sexual and vitality/hormonal domain scores.
All domains for EPIC-26 are reported on a 0 to 100 score, with higher scores representing favorable Health-Related Quality of Life (HRQOL).
