- 참가자가 본 연구를 이해할 수 있는 능력 및 서면 동의서에 서명하려는 참가자의 의지
- 18세 이상 남성 및 여성
- RECIST 1.1에 따라 측정 가능한 질병 참가자는 RECIST v1.1 기준에 따라 측정할 수 있는 방사선 사진으로 식별 가능한 병변이 2개 이상 있어야 하며, 해당 병변 중 하나는 65cc 이상이어야 합니다. 참고: 'cc'는 대략적인 근사치를 제공하기 위해 (너비 x 길이 x 높이)/2와 같습니다. 예를 들어, 5.1 x 5.1 x 5.1cm 크기의 종양은 66.3cc이며 시험 등록에 적합합니다.
- 참가자는 마지막 전신 치료 시점부터 연구 치료 시작 시점까지 5회의 제거 반감기 또는 28일(둘 중 더 긴 기간)의 휴약 기간을 가져야 합니다.
- 가임 여성은 등록 시(스크리닝 시 및 방사선 요법 또는 면역 요법 시작 전 최대 48시간까지) 혈청 임신 검사에서 음성 반응을 보여야 합니다.
- 생검 및 방사선학적으로 확인된 전이성 암(예: 폐암, 두경부암, 난소암, 대장암, 육종)이 있는 참가자
- 참가자에 대해 최소 1차(1L) 전신 요법 또는 면역요법이 실패해야 하며 표준 치료 요법 옵션도 소진되어야 합니다. 질병 진행 이외의 이유로 중단된 표준 치료 옵션은 적격합니다. 참고: 감시 영상으로 모니터링되고 있는 전신 치료를 받지 않은 참가자는 질병 진행이 발생하고 표준 치료 요법 옵션이 없는 참가자는 등록할 수 있습니다.
- 최소 하나의 병변(최대 4개의 병변)은 65cc 이상이어야 합니다(종양 길이 x 너비 x 높이를 사용하여 가장 가까운 근사치로 계산).
- 초고선량 SBRT를 사용하여 치료할 병변과 주변 영역(필요에 따라 사례별로 평가할 수 있는 상당한 선량 중복 없음)이 이전에 치료되지 않은 경우 사전 RT가 허용됩니다.
- CNS 전이성 질환이 있는 참가자는 CNS 외 절제 요법을 시작하기 전에 모든 병변을 관리하는 경우 프로토콜에 허용됩니다.
- 탐색적 연구 관련 바이오마커 연구를 위한 연구 개입 이전에 수행된 암의 전이 진단에서 대표(FFPE) 종양 표본의 가용성
연구 치료 시작 전 14일 이내에 얻은 다음 실험실 테스트 결과로 정의되는 적절한 기관 기능:
- 백혈구 ≥ 3K/μL
- 림프구 수 ≥ 0.5 x 10^9/L(500/μL)
- 과립구 집락 자극 인자 지원 없이 절대 호중구 수 ≥1.5K/μL. 참고: 양성 호중구 감소증으로 진단된 참가자는 담당 의사의 의견에 따라 시험 치료가 참가자에게 과도한 감염 위험을 초래하지 않는 경우 1000~1500 사이에 ANC에 참여할 수 있습니다.

- 헤모글로빈 ≥ 9g/dL

가임 여성 및 가임 파트너가 있는 남성은 연구 참여 기간 및 180일 동안 성적 금욕을 실천하거나 가임 가능성/임신 섹션 및 아래에 나열된 피임법을 사용하는 데 동의해야 합니다. 치료 완료 후 6개월. 남성은 같은 기간 동안 정자 기증을 삼가해야 합니다.
- 여성은 초경 후이고, 폐경기 상태(폐경 이외의 확인된 원인 없이 12개월 연속 무월경이 지속됨)에 도달하지 않았으며, 외과적 불임수술(난소 제거, 나팔관 제거 및 난관 제거)을 받지 않은 경우 가임 가능성이 있는 것으로 간주됩니다. 또는 자궁) 또는 연구자가 결정한 다른 원인(예: 뮐러식 무형성). 이 정의에 따르면, 난관 결찰을 받은 여성은 가임 가능성이 있는 것으로 간주됩니다. 가임 능력의 정의는 현지 지침이나 요구 사항에 맞춰 조정될 수 있습니다.

- 금욕의 신뢰성은 임상 시험 기간과 참가자가 선호하는 일반적인 생활 방식과 관련하여 평가되어야 합니다. 주기적인 금욕(예: 달력법, 배란법, 증상체온법, 배란 후 방법)과 금욕은 적절한 피임법이 아닙니다. 현지 지침이나 규정에 따라 필요한 경우, 현지에서 인정된 적절한 피임 방법과 금욕의 신뢰성에 대한 정보가 현지 사전 동의서에 설명됩니다.
제외 기준:
- 모든 치료 임상 시험에 동시에 등록
- 본 연구에 참여하는 동안 다른 항종양제 또는 연구용 약제를 현재 사용 중이거나 사용 예정인 경우
- 정신 질환 진단을 받았거나 연구 요구 사항 준수를 제한하는 사회적 상황에 있는 경우
- 임신 중이거나 수유 중인 경우
- 연구 치료제를 받는 동안 또는 연구 치료제의 마지막 투여 후 180일/6개월 미만 동안 임신을 계획하고 있는 가임기 여성. 가임 여성은 등록 시(스크리닝 시 및 방사선 요법 또는 면역 요법 시작 전 최대 48시간까지) 혈청 임신 검사 결과가 음성이어야 합니다.
- 연구 치료제를 받는 동안 또는 연구 치료제의 마지막 투여 후 180일/6개월 미만 동안 아이를 낳거나 정자를 기증할 계획을 갖고 있는 가임기 남성

- 골전이성 질환만 있는 참가자
- 면역억제 장애(예: 고형 장기 이식 수혜자, 동종 줄기 세포 이식)(전체 목록은 부록 D 참조)
- 호스피스에 등록되어 있거나 기대 수명이 6개월 미만이라고 생각되는 참가자
통제되지 않거나 치료되지 않은 CNS 전이
- 증상이 있거나 치료되지 않았거나 활발하게 진행 중인 중추신경계(CNS) 전이
치료된 CNS 병변이 있는 무증상 참가자는 다음 기준을 모두 충족하는 경우 자격이 있습니다.
- RECIST v1.1에 따라 측정 가능한 질병은 CNS 외부에 존재해야 합니다.
- 참가자는 두개내 출혈이나 척수 출혈의 병력이 없습니다.
- 참가자는 연구 치료 시작 전 7일 이내에 정위 방사선요법을 받지 않았으며, 연구 치료 시작 전 14일 이내에 전뇌 방사선요법을 받지 않았거나, 연구 치료 시작 전 28일 이내에 신경외과적 절제술을 받지 않았습니다.
- 참가자는 CNS 질환에 대한 치료법으로 코르티코스테로이드가 지속적으로 필요하지 않습니다.
- 참가자가 항경련제 치료를 받고 있는 경우 용량은 안정적인 것으로 간주됩니다.
- 연수막병의 병력
- 조절되지 않는 흉막삼출, 심낭삼출 또는 반복적인 배액 절차(월 1회 이상)가 필요한 복수. • 유치 카테터(예: PleurX)를 사용하는 참가자는 허용됩니다.
- 조절되지 않거나 증상이 있는 고칼슘혈증(이온화 칼슘 > 1.5mmol/L, 칼슘 > 12mg/dL 또는 교정된 혈청 칼슘 > ULN
- 조절되지 않는 종양 관련 통증. 진통제가 필요한 참가자는 연구 시작 시 안정적인 처방을 받아야 합니다.
중증근육무력증, 근염, 자가면역 간염, 피부경화증, 전신홍반루푸스, 류마티스 관절염, 염증성 장질환, 항인지질항체증후군, 베게너 육아종증, 쇼그렌 증후군, 길랭- 바레 증후군 또는 다발성 경화증(다음 예외 포함):
- 자가면역 관련 갑상선 기능 저하증 병력이 있고 갑상선 대체 호르몬을 복용 중인 참가자는 연구에 참여할 수 있습니다.
- 인슐린 요법을 받고 있는 제1형 당뇨병이 조절된 참가자는 연구에 참여할 수 있습니다.
습진, 건선, 만성 단순 태선 또는 피부과적 징후만 있는 백반증이 있는 참가자(예: 건선성 관절염이 있는 참가자는 제외)는 다음 조건이 모두 충족되는 경우 연구에 참여할 수 있습니다.
- 발진은 신체 표면적의 < 10%를 차지해야 합니다.
- 질병은 기준선에서 잘 조절되며 효능이 낮은 국소 코르티코스테로이드만 필요합니다.
- 지난 12개월 이내에 솔라렌과 자외선 A 방사선, 메토트렉세이트, 레티노이드, 생물학적 제제, 경구용 칼시뉴린 억제제, 고효능 또는 경구용 코르티코스테로이드를 필요로 하는 기저 질환의 급성 악화가 발생한 적이 없습니다.

- 특발성 폐섬유증, 조직성 폐렴(예: 폐쇄성 세기관지염), 약물 유발성 폐렴 또는 특발성 폐렴의 병력, 또는 선별 흉부 컴퓨터 단층촬영(CT) 스캔에서 활동성 폐렴의 증거. 방사선 분야 방사선 폐렴(섬유증)의 병력은 허용됩니다.
- 활동성 결핵
- 연구 치료 시작 전 3개월 이내에 유의한 심혈관 질환(예: 뉴욕심장협회 Class II 이상의 심장 질환, 심근경색 또는 뇌혈관 사고), 불안정 부정맥 또는 불안정 협심증
- 연구 치료 시작 전 4주 이내에 진단을 위한 이외의 대수술 절차, 또는 연구 기간 동안 대수술이 필요할 것으로 예상되는 경우
- 연구 치료 시작 전 4주 이내에 심각한 감염(감염 합병증, 균혈증 또는 중증 폐렴으로 인한 입원, 또는 참가자 안전에 영향을 미칠 수 있는 모든 활동성 감염을 포함하되 이에 국한되지 않음)
- 연구 치료 시작 전 2주 이내에 치료용 경구 또는 IV 항생제로 치료. 예방적 항생제(예: 요로 감염 또는 만성 폐쇄성 폐질환 악화를 예방하기 위해)를 받는 참가자는 연구에 참여할 수 있습니다.
- 시험약의 사용을 금하는 기타 질병, 대사 장애, 신체 검사 소견 또는 임상 실험실 소견은 결과 해석에 영향을 미치거나 참가자를 치료 합병증의 위험이 높게 만들 수 있습니다.
- 연구 치료 시작 전 4주 이내에 약독화 생백신으로 치료하거나, 아테졸리주맙 치료 중 또는 아테졸리주맙 최종 투여 후 5개월 이내에 그러한 백신이 필요할 것으로 예상되는 경우
- HBV에 대한 항바이러스 요법을 이용한 현재 치료법
- 연구 치료 시작 전 28일 이내에 연구 요법을 통한 치료
- 체크포인트 차단 요법을 통한 사전 치료
- 이전 면역요법으로 인한 폐렴 및/또는 등급 ≥3인 복막염의 병력은 허용되지 않습니다(완전히 회복되고 후속 면역요법에 내약성이 있는 등급 1 또는 2의 경우는 허용됨)
- 연구 치료 시작 전 4주 또는 약물 반감기 5회(둘 중 더 긴 기간) 이내에 전신 면역자극제(인터페론 및 인터루킨 2[IL-2]를 포함하되 이에 국한되지 않음)로 치료
연구 치료 시작 전 2주 이내에 전신 면역억제제(코르티코스테로이드, 시클로포스파미드, 아자티오프린, 메토트렉세이트, 탈리도마이드 및 항TNF-α 제제를 포함하되 이에 국한되지 않음)로 치료했거나, 연구 기간 동안 전신 면역억제제가 필요할 것으로 예상되는 경우 다음과 같은 예외를 제외하고 연구 치료제:
- 급성, 저용량 전신 면역억제제 또는 전신 면역억제제의 1회 펄스 투여(예: 조영제 알레르기에 대한 48시간 코르티코스테로이드)를 받은 참가자는 연구에 참여할 수 있습니다.
- 미네랄코르티코이드(예: 플루드로코르티손), 만성 폐쇄성 폐질환(COPD)이나 천식을 위한 코르티코스테로이드, 기립성 저혈압이나 부신 기능 부전을 위한 저용량 코르티코스테로이드를 투여받은 참가자는 연구에 참여할 수 있습니다.
