ACHTERGROND:
Naarmate medische behandelingen voor coronaire hartziekten geavanceerder zijn geworden, zijn ze ook duurder geworden. Bewijs met betrekking tot de effecten van medische en revalidatietherapieën op de kwaliteit van leven van patiënten na een myocardinfarct, inclusief terugkeer naar werk en normale niveaus van functioneren, is gemengd. Tegelijkertijd. recente gegevens suggereren dat psychosociale factoren, zoals sociaal isolement en depressie, belangrijke voorspellers zijn van morbiditeit en mortaliteit bij patiënten met coronaire hartziekte. Deze studies suggereren dat interventies die psychologische ondersteuning bieden aan myocardinfarctpatiënten zowel het psychosociale als fysieke herstel van deze patiënten kunnen bevorderen. Voor zover kan worden aangetoond dat ondersteunende interventies een gunstig effect hebben op de overleving en de gezondheidsgerelateerde kwaliteit van leven bij patiënten met een hartinfarct, kunnen de menselijke en financiële kosten die gepaard gaan met coronaire hartziekten worden verminderd. Het initiatief kwam voort uit de Werkgroep Psychosociale Interventies die in juni 1992 bijeenkwam.
Het initiatief kreeg conceptgoedkeuring van de National Heart, Lung and Blood Advisory Council van oktober 1993. Het verzoek om voorstellen werd uitgebracht in september 1994.
ONTWERP VERHAAL:
De onderzoeksopzet vergeleek een psychosociale interventiegroep, waarin patiënten sociale en psychologische behandeling kregen om sociaal isolement en depressie te verminderen, met een standaard medische zorggroep. Het gecombineerde eindpunt was overlijden en opnieuw infarct, gemeten tot 4,5 jaar na ontslag uit het ziekenhuis. Secundaire eindpunten omvatten beoordeling van de gezondheidskwaliteit van leven (HQL).
Het protocol werd in mei 1996 goedgekeurd. De rekrutering begon in oktober 1996 en eindigde op 31 oktober 1999 met 2.481 ingeschreven patiënten.
