背景:目前出院的老年患者经历了零散的护理、反复和长时间的急诊科 (ED) 就诊、病情复发以及认知和功能迅速下降。 西奈山医院的老年人急症护理 (ACE) 计划使用过渡教练、随访电话和患者自我护理指南等创新策略,改善体弱老年患者从医院到社区的护理过渡体验。 2014 年,ACE 计划缩短了老年患者的住院时间和再入院时间,提高了患者满意度,并为医疗保健系统节省了超过 600 万美元。

Zarit 负担访谈 (ZBI) 是衡量护理人员负担的最常用工具之一。
