Project Rise er et randomiseret kontrolleret forsøg (RCT) af en innovativ, kulturelt kongruent behandlingsuddannelse (TE) intervention for afroamerikanere med hiv, der retter sig mod kulturelle og sociale problemer, der bidrager til sundhedsforskelle. TE letter patientnavigation gennem det medicinske system og tilbyder behandlingsuddannelse og klientcentreret rådgivning for at forbedre overholdelse og fastholdelse i plejen. TE retter sig mod strukturelle problemer i sundhedsvæsenet og patienternes liv ved at opfordre udbydere til at forbedre forholdet mellem patient og udbyder, anbefale ændringer i behandling og/eller udbydere og henvise patienter til mental sundhed og sociale tjenester.
I alt 350 sorte deltagere vil blive rekrutteret gennem APLA Health og tilfældigt tildelt interventions- eller sædvanlig plejekontrolgruppe (175 pr. gruppe). Overholdelse vil blive overvåget elektronisk dagligt (og downloadet hver anden måned) fra baseline til 13 måneder efter baseline. Viral belastning vil blive vurderet ved venepunktur ved baseline og 6- og 13 måneder efter baseline. Det er en hypotese, at interventionsgruppen vil vise bedre adhærens og have en større sandsynlighed for undertrykt virusbelastning end kontrolgruppen over tid.

Inklusionskriterier:
- HIV-positive
- 18 år eller ældre
- Selvidentificeret som afroamerikaner/sort
- Har fået ordineret antiretroviral behandling (ART) inden for de seneste 12 måneder
- Selvrapporterede overholdelsesproblemer (dvs. glemte mindst 1 ART-dosis inden for den seneste måned) og/eller påviselig viral belastning
- Villig til at bruge Medication Events Monitoring System (MEMS) til elektronisk overholdelsesovervågning.
Ekskluderingskriterier:
- HIV-negativ
- 17 år eller yngre
- Ikke selv-identificeret som afroamerikaner/sort, ikke i antiretroviral behandling (ART) eller ikke ordineret ART-behandling inden for de sidste 12 måneder
- Ingen selvrapporterede adhærensproblemer og/eller ingen påviselig viral belastning
- Ikke villig til at bruge Medication Events Monitoring System (MEMS) til elektronisk overholdelsesovervågning.

Eksperimentel: Rise - Behandlingsuddannelse
Rise består af en én måneds intensiv intervention (med tre centrale 60-minutters rådgivningssessioner i uge 1, 2 og 4) efterfulgt af to booster-sessioner (i uge 12 og 20).
Hvis deltagerne viser manglende overholdelse under booster-sessioner, tilbydes de op til fire ekstra booster-sessioner (dvs. ekstra booster-sessioner, hvis <85 % af de ordinerede doser blev taget inden for den seneste måned).
Deltagerne får således tre kernesessioner i den første måned, efterfulgt af 2-6 boostersessioner over de næste fire måneder.
