Descripción detallada
This was a multinational, multicenter, parallel-group, prospective, randomized, open-label, with blinded-endpoint adjudication, active-controlled study. The target projected sample size was approximately 900 participants (450 in each arm). It was estimated that approximately 1800 participants would be screened at sites in Latin America, including Argentina, Brazil, Colombia, and Mexico.
Participants who met the eligibility requirements were randomly assigned in a 1:1 ratio to receive sacubitril/valsartan (target dose of 200 mg twice daily) or enalapril (target dose of 10 mg twice daily), in addition to their usual therapy, stratified by country, using a central, concealed, web-based, automated randomization system. Both groups entered a titration period of 3 to 6 weeks, aiming to achieve the target dose of sacubitril/valsartan 200 mg twice daily or enalapril 10 mg twice daily. The study follow-up succeeded the titration period and was to last until a total number of 302 events was reached and all randomized participants had a minimum follow-up of 12 weeks.

Criterios clave de inclusión:
- Hombre o mujer ≥ 18 años de edad
Diagnóstico de HFrEF clase II-IV de la NYHA establecido por:
- FEVI ≤ 40 % dentro de los 12 meses anteriores a la visita 1 realizada mediante cualquier medición local mediante ecocardiografía, escaneo de adquisición múltiple sincronizada (MUGA), tomografía computarizada (TC), resonancia magnética (RM) o angiografía ventricular, siempre que no haya una medición posterior anterior 40% Y
- NT-proBNP ≥ 600 pg/mL (o BNP ≥ 150 pg/mL) en la Visita 1 O
- NT-proBNP ≥ 400 pg/mL (o BNP ≥ 100 pg/mL) en la Visita 1 y una hospitalización por IC en los últimos 12 meses
- Diagnóstico de enfermedad de Chagas confirmado por al menos dos pruebas serológicas diferentes para anti-Trypanosoma cruzi basadas en diferentes principios o con diferentes preparaciones antigénicas, tales como: ensayo inmunoabsorbente ligado a enzimas [ELISA], inmunofluorescencia indirecta [IFI], hemaglutinación indirecta [IHA] , western blot (WB), inmunoensayo quimioluminiscente (CLIA). Si el historial documentado no está disponible, las pruebas se pueden realizar durante la evaluación.
Criterios clave de exclusión:
- Pacientes con antecedentes de angioedema sospechado o comprobado o que no pueden tolerar los IECA, ARB o ARNI (p. ej., debido a tos, hipotensión, disfunción renal, hiperpotasemia)
- Uso de sacubitrilo/valsartán en los últimos 3 meses
Pacientes que requieran tratamiento inotrópico intravenoso continuo o con indicación de intervención avanzada de soporte para IC:
- ya en lista para un trasplante de corazón
- con indicación actual de dispositivo de asistencia ventricular izquierda o terapia de resincronización cardíaca (TRC)
- Presión arterial sistólica sistémica inferior a 95 mmHg o hipotensión sintomática
- Potasio sérico > 5,2 mmol/L
- Tasa de filtración glomerular estimada (TFGe) < 30 ml/min/1,73 m2 de superficie corporal
- Forma gastrointestinal grave de la enfermedad de Chagas crónica (megaesófago demostrado y/o megacolon importante, por ejemplo: con ingesta oral comprometida o indicación quirúrgica).
- Condiciones clínicas o enfermedades sistémicas que limitan la participación adecuada del paciente
- Mujeres embarazadas o lactantes o mujeres en edad fértil a menos que estén usando métodos anticonceptivos altamente efectivos
Presencia de otras condiciones cardíacas:
- Cirugía cardiaca previa
- Insuficiencia cardíaca cuando, a juicio del Investigador, existe una posible etiología primaria alternativa, por ejemplo, debido a enfermedad de las arterias coronarias, enfermedad de las válvulas, enfermedad cardíaca congénita u otras causas.
- Arritmia no tratada o enfermedad grave de la conducción, por ejemplo, bradiarritmias, fibrilación auricular con respuesta ventricular rápida, bloqueo auriculoventricular de segundo o tercer grado, etc.
- Patología valvular primaria no corregida como estenosis aórtica de moderada a grave, estenosis mitral e insuficiencia mitral primaria
- Trasplante de órganos planificado (o en la lista para trasplante), cirugía cardíaca u otra cirugía mayor planificada (incluido el implante de un dispositivo de asistencia ventricular)
- Antecedentes de malignidad de cualquier sistema orgánico en los últimos 5 años.
- Infección actual confirmada por COVID19
- Infección previa por COVID19 con carga de síntomas persistentes sospechosa debido a COVID19 (los síntomas persistentes pueden incluir, entre otros, tos continua, dificultad para respirar, dolores musculares/articulares y síntomas gastrointestinales desde el momento de la infección por COVID19 en adelante)

Experimental: Sacubitril/valsartan

Participants randomized to sacubitril/valsartan who were previously treated with angiotensin-converting enzyme inhibitors (ACEIs) had a 36-hour washout prior to receiving oral treatment with 50, 100, or 200 mg, film-coated tablets.

The primary efficacy endpoint was analyzed using the win ratio approach comparing every participant in the sacubitril/valsartan arm to every participant in the enalapril arm to determine a winner.
A winner in the pair-wise comparison had a delayed time to the occurrence of CV death; if time to the occurrence of CV death was censored, a winner had a delayed time to the occurrence of first HF hospitalization event; if the times to both CV events were censored, a winner had a more favorable (less increase or more decrease) change in NT-proBNP between Baseline and Week 12.
The estimated win ratio was defined as the total number of winners in the sacubitril/valsartan arm divided by the total number of winners in the enalapril arm.
A win ratio >1 represents a favorable outcome for the study drug being assessed.
