Este ensayo clínico multicéntrico tiene como objetivo investigar la eficacia y la seguridad de la acupuntura manual en el tinnitus primario agudo.

Descripción detallada
Este es un ensayo multicéntrico, aleatorizado, simple ciego y controlado con simulación.
El período total del ensayo fue de 14 semanas, incluyendo dos semanas de evaluación basal, cuatro semanas de tratamiento después de la aleatorización y ocho semanas de seguimiento.
Se obtendrá el consentimiento informado de cada paciente antes de la aleatorización.
Después de la evaluación basal, un investigador independiente aleatorizará a los pacientes elegibles en una proporción 1:1:1 para recibir acupuntura manual, acupuntura simulada o atención habitual.
Todos los pacientes recibirán 10 sesiones de 30 minutos de tratamientos de acupuntura o atención habitual durante un período de tratamiento de cuatro semanas.
Serán tratados tres veces por semana para completar un curso de tratamiento inicial de dos semanas y luego serán tratados dos veces por semana para completar un curso de tratamiento de consolidación de dos semanas.
Tanto en el grupo de acupuntura manual como en el de acupuntura simulada, el tratamiento de acupuntura comenzará después de la aleatorización.
En el grupo de atención habitual, los pacientes recibirán acupuntura de forma gratuita después de esperar 12 semanas.

Criterios de inclusión:
- Edad comprendida entre 18 y 65 años (cualquier sexo);
- La queja principal es el tinnitus;
- Cumple los criterios diagnósticos para tinnitus primario;
- El tinnitus debe ser molesto, con una puntuación THI entre 28 y 76, y durar ≥2 semanas;
- Duración del tinnitus inferior a 6 meses;
- Sin experiencia con acupuntura en los últimos 3 meses;
- No haber usado medicación para el tinnitus dentro de las 2 semanas previas al tratamiento, incluyendo inyecciones intratimpánicas, inyecciones retroauriculares, corticosteroides, Medicina Tradicional China (MTC) o medicamentos patentados chinos, etc.;
- Aceptar firmar el formulario de consentimiento informado.
Criterios de exclusión:
- Tinnitus objetivo;
- Tinnitus pulsátil;
- Diagnosticado como tinnitus secundario según evaluación de audiólogos y clínicos;
- Pérdida auditiva conductiva combinada (confirmada por otoscopia anormal o timpanograma anormal);
- Contraindicaciones para la acupuntura, incluyendo mujeres embarazadas, anemia grave, trastornos de coagulación, etc.;
- Dificultades para expresar claramente sus síntomas, como trastornos mentales graves o deterioro cognitivo;
- Padecer otras enfermedades orgánicas agudas o crónicas graves actualmente, como enfermedades cerebrovasculares, cardiovasculares, hepáticas, renales, hematológicas, infecciosas o tumores malignos;
- Participación en otros ensayos clínicos, especialmente aquellos relacionados estrechamente con el tratamiento del tinnitus, como la terapia sonora;
- Tener un fuerte deseo de tratamiento farmacológico.

The pittsburgh Sleep Qual-ityIndex (PSQI) is a self-rated questionnaire which assesses sleep quality and disturbances over a 1-month time interval.
It has 19 questions which can be grouped into seven categories: subjective sleep quality, sleep latency, sleep duration, habitual sleep efficiency, sleep dis-turbances, use of sleeping medication, and daytime dysfunction.The PSQI total score ranges from 0 to 21, with higher scores indicating worse sleep quality.

Adverse events assessed each acupuncture treatment visit using a acupuncture adverse events questionnaire recording presence and severity of side effects (e.g., subcutaneous hemorrhage, minor bleeding) related to acupuncture, plus event frequency.

Massachusetts General Hospital Acupuncture Sensation Scale(MASS)

The Massachusetts General Hospital Acupuncture Sensation Scale, which includes twelve specific descriptors-soreness, aching, deep pressure, heaviness, fullness or distension, tingling, numbness, sharp pain, dull pain, warmth, cold, and throbbing-as well as an open field for patients to describe the sensation in their own words, will be administered following a brief explanation of de qi sensations by the acupuncturist to participants in the verum acupuncture and sham acupuncture groups, while the usual care group will not receive this questionnaire.

Auditory Sensitivity Scale

Auditory Sensitivity Scale is a self-report questionnaire consisting of general questions on tinnitus and sound intolerance, along with 25 items rated on different scoring scales (e.g., 0/2/5, 0/1/2, 0/10), and provides a severity classification based on the total score: 31-55 for mild, 56-80 for moderate, 81-100 for severe, and 101-126 for very severe.

Pure-Tone Audiometry(PTA)

|
Pure tone audiometry is conducted under standard soundproof conditions using the Hughson-Westlake method to obtain the air conduction threshold (bone conduction threshold is only used for diagnosis, to determine whether there is conductive hearing loss, and to assist in the screening process; statistical analysis only uses the air conduction threshold).
The standard test frequencies range from 0.25 to 8 kHz.
If conditions permit, extended high frequencies (Extended High Frequency, EHF; ≥ 9-16 kHz) are added, and necessary masking is performed in accordance with the norms.
