Probioottisten lisäravinteiden turvallisuus ja tehokkuus kaksisuuntaisessa masennuksessa: konseptin todiste satunnaistettu kontrolloitu tutkimus
Tämä on 8 viikkoa kestänyt satunnaistettu, kaksoissokkoutettu, lumekontrolloitu proof of concept -tutkimus, jossa arvioidaan mielialan stabilointiaineen + Tasapainotuksen yhdistelmää potilaiden hoidossa, joilla on kaksisuuntainen mielialahäiriö. Tutkimuksessa on kaksi hoitohaaraa: mielialan stabilointiaine plus lumelääke ja mielialan stabilointi sekä Align.
Mielialan stabilointiaineen annos on kliinisen käytännön ohjeiden mukainen ja Align-annos on 1 kapseli päivässä sopivan tuotteen annostelun mukaan.

Yksityiskohtainen kuvaus
Masennus on noussut suurimmaksi haasteeksi kaksisuuntaisen mielialahäiriön lyhyen ja pitkän aikavälin hoidossa; tarvitaan uusia hoitoja tämän taudin hallitsemiseksi. Tutkimukset osoittavat, että vatsassa olevat probiootit parantavat mielialaa ja vähentävät ahdistuneisuusoireita, mikä tukee edelleen käsitystä, että probioottibakteerit vaikuttavat positiivisesti tunnetiloihin. Ehdotamme mielialan stabilointiaineen ja probiootin Alignin yhdistelmän tehokkuuden arvioimista mielialan stabilointiainemonoterapiaan verrattuna.
Tämän toteutettavuustutkimuksen ensisijaisena tavoitteena on määrittää Alignin turvallisuus ja siedettävyys lisänä BD:ssä hyväksyttyyn monoterapiaan. Toissijainen tavoite: 1) Arvioida mielialan stabilointiaineen + Alignin yhdistelmän tehokkuutta mielialan stabilointiainemonoterapiaan verrattuna BD:n hoidossa. 2) sen määrittämiseksi, vähentääkö Align + mielialan stabilointiyhdistelmä ahdistuneisuusoireita.
Kahdeksan viikkoa kestänyt satunnaistettu, kaksoissokkoutettu lumekontrolloitu proof of concept -tutkimus, jossa arvioitiin mielialan stabilointiaineen + Alignin yhdistelmää potilaiden hoidossa, joilla on kaksisuuntainen mielialahäiriö. Tutkimuksessa on kaksi hoitohaaraa: mielialan stabilointi + lumelääke ja mielialan stabilointi + Align. Osallistujat arvioidaan ja heille annetaan tutkimusasteikot seulonnassa, lähtötilanteessa ja viikoilla 2, 4, 6 ja 8. mielialan stabiloijan annos on kliinisen käytännön ohjeiden mukainen ja Align - 1 kapseli/vrk.

- Mies- ja naispuoliset osallistujat 18-65-vuotiaat seulontahetkellä mukaan lukien
- Osoittaa kaksisuuntaisen mielialahäiriön (tyyppi I tai II) diagnoosin Mini International Neuropsychiatric -haastattelun (MINI) mukaan
- Ei ole sairaalahoidossa tai laitoshoidossa (avohoidossa) seulonnan aikana
- olet ollut vakaalla (muuttumattomalla) ja riittävällä annoksella jotakin sallituista lääkkeistä vähintään 8 viikon ajan ennen seulontaa (litium, valproaatti/divalproaatti, karbamatsepiini, lamotrigiini, ketiapiini/ketiapiini XR, olantsapiini tai risperidoni)
- Osoittaa maksimi HAM-D-pisteen 24 seulonta- ja peruskäynneillä
- Naispuolisten osallistujien tulee:

Poissulkemiskriteerit:
- Osoittaa tai raportoi jonkin DSM-IV:n kriteerin päihteiden väärinkäytön/riippuvuushäiriöiden luokittelun historiasta viimeisen vuoden aikana.
- Osoittaa aktiivisten syömishäiriöiden, skitsofrenian tai skitsoaffektiivisen häiriön olemassaolon
- Osoittaa nykyisiä psykoottisia oireita
- Osoittaa Young Mania Rating Scale (YMRS) -pistemäärän yli 12 seulonnassa
- Osoittaa aktiivista itsemurhaa C-SSR-asteikon perusteella
- Raportit, joissa on käytetty voimakkaita sytokromi P450:n estäjiä (kuten kloramfenikolia, sienilääkkeitä) seulontaa edeltäneiden 14 päivän aikana
- Raportoi sähkökouristushoidon (ECT) historian 90 päivän aikana ennen seulontaa
- Raportit, joissa on käytetty Alignia tai mitä tahansa muuta probioottista lisäravinnetta viimeisen vuoden aikana ennen seulontaa
- Raportoi aiemmista allergisista reaktioista Alignin komponentteihin (kuten Bifidobacterioum) tai muihin vastaaviin lääkkeisiin.
- Raportoi lääkkeiden farmakokinetiikkaan vaikuttavien sairauksien esiintymisestä tai historiasta (ruoansulatuskanavan sairaudet, kuten chroris tai kolitus).
- Raportoi aiemmista immuunijärjestelmää heikentävistä tiloista (kuten HIV, lymfooma, potilaat, jotka saavat pitkäaikaista kortikosteroidihoitoa)
- sinulla on tällä hetkellä pahoinvointia, kuumetta, oksentelua, veristä ripulia tai voimakasta vatsakipua
- Raportoi muiden masennukseen vaikuttavien luonnollisten terveystuotteiden (kuten Johns Wart, Tryptophan) käytön (kuten probioottien lisäksi) viimeisten 6 kuukauden aikana ennen seulontaa
- Raportoi usein probiootteja sisältävien/rikastettujen elintarvikkeiden (esim. jogurtti, juomat jne.).

Siedettävyys arvioidaan Toronton sivuvaikutusasteikolla (TSES).
Se on 32-osainen instrumentti, joka on suunniteltu keskushermoston (CNS), maha-suolikanavan (GI) ja seksuaalisten sivuvaikutusten esiintyvyyden, esiintymistiheyden ja vakavuuden määrittämiseen.

Ahdistuneisuusoireita arvioidaan tarkastelemalla tila- ja ominaisuus-ahdistusindeksin (STAI) kokonaispistemäärän keskimääräistä muutosta, elämänlaatua arvioidaan Maailman terveysjärjestön WHO-QOL-asteikolla ja tarkastellaan globaalia toimintaa. seuraavien keskimääräisten muutosten kautta: Sheehan Disability Scale (SDS) -pisteet, kliiniset globaalit vaikutelmat (CGI) sairauden vakavuuspisteet, CGI-parantumisasteikko ja CGI Participant Assessment.
